Across the contiguous 48 states, populations in states with more activist civic cultures have lower mortality than states that do not follow this model. Several different factors can be pointed to as causes for this discrepancy - net income, class inequality, and the history of settlement in each of the different states and regions. These observations are true of Non-Hispanic Whites and African Americans but not of American Indians, and Hispanics, neither of which is fully integrated into the state political culture and economy in which it resides. In Regional Cultures and Mortality in America, the struggles these various populations face in regard to their health are explored in terms of where they reside.

Each of the book's four sections includes an introduction that contextualizes the issues and addresses the larger stakes of his work. An afterword discusses the significance of Grossman's approach for subsequent research on health economics, as well as the work others have done to advance and extend his innovative perspective.Determinants of Health explains how the economic choices people make influence health and health behaviors. It begins with a section on the theoretical underpinnings and empirical results of Grossman's groundbreaking health economics model, first introduced in the 1970s, followed by essays on the relationship between health and schooling; determinants of infant health, with a special emphasis on public policies and programs; and the economics of unhealthy behaviors. Grossman treats health as a form of human capital. He shows that public policies and programs that determine the price and availability of key inputs have critical effects on outcomes ranging from birth weight and infant mortality to cigarette smoking, alcohol abuse, illegal drug use, and obesity. This book investigates several important issues in the economics of aging, including the accumulation of wealth and the relationship between health and financial prosperity. Examining the changes in savings behavior and investment priorities in the United States over the past few decades, contributors to the volume point to a dramatic shift from employer-managed, defined benefit pensions to employee-controlled retirement savings plans. Further, the legislative reforms of the 1980's and the booming stock market of the 1990's did their share to influence individual wealth accumulation patterns of Americans. These studies also explore the relationship between health status and economic status. Considering issues like pension income and health, mortality, and medical care, contributors present evidence from the United States, Britain, South Africa, and Russia. The volume culminates with wide-ranging discussions on a number of key topics in the field including the innovations that have contributed to a decline in mortality rates; the various medical advances that have benefited populations over time; and the determinants of expenditures on health. The findings with regard to cross-sectional differences in health outcomes and health care utilization also pose troubling questions for policymakers seeking to democratize health care across regions and races.
