Abstract

The surgical treatment of hernia requires an extensive knowledge and technical ability. Astley Cooper stated that no disease of surgical interest requires so broad skills and knowledge as hernia and its variants. The history of groin hernia repair evolved from life-saving procedures (such as for incarcerated hernias) to elective operations performed as day-surgery procedure. The present book is designed to focus on specific topics and problems which a general surgeon dealing with groin hernia is very likely to face during his practice. Le traitement de la hernie discale, objectivée médicalement, dans notre société industrielle, semble impliquer comme solution finale quasi obligatoire habituelle, un acte chirurgical, ce qui nous parait excessif. N'existerait-il pas une autre alternative à cette décision ?Ces années nous ont donnés avec plus de 400 cas traités, une pratique suffisante. Accompagnée d'un perfectionnement technique progressif, cette dernière nous permet d'avoir la certitude absolue de son efficacité d'une part, ainsi que de sa sécurité absolue, d'autre part. Ce qui m'autorise à vous en faire part aujourd'hui, afin d'en partager sa pratique.Bien sûr je n'ignore pas qu'en science médicale, tant que l'on n'a pas établi un protocole, on ne peut accepter pleinement les observations d'un simple praticien. Ce qui priverait celui-ci de pouvoir contribuer aux connaissances d'un art qu'il pratique et défend. Mais serait-il éthique de garder pour soi une mise au point technique qui s'avère absolument sûre, simple et efficace ? En fait, nous avons seulement suivi, les lois générales préconisées par F. BACON qui, à partir des faits, établissent la méthode connue comme induction, avec ses quatre étapes successives que sont l'observation des faits, puis leur analyse et leur classification, ce qui permet alors par induction des généralisations qui en donnent l'explication. A la fin, de plus, on peut affirmer qu'une explication n'est valide que si elle se vérifie, c'est à dire si, dans ce cas la technique crée bien les conditions pour une guérison du patient. Car, seul le corps a réellement cette capacité.
