The geriatric population requires careful monitoring of this medications. Many physiological parameters are modified, and they play a role in the introduction of the drug in the body. A patient’s overall care is needed. In this brief, we evaluated the introduction of the medication history, done in hospital by clinical pharmacists in pharmacies open to the public. The analysis of this history was performed using validated tools and present in the literature. The result was then synthesized by the production of a medication regimen given to the patient. Finally, these steps have allowed us to highlight the strengths and weaknesses of this approach in pharmacies in order to improve patient treatment in collaboration with the doctor. La population gériatrique nécessite un suivi rigoureux de son traitement médicamenteux. De nombreux paramètres physiologiques sont modifiés et ceux-ci jouent un rôle lors de l'introduction du médicament dans l'organisme. Une prise en charge globale du patient est donc nécessaire.Dans ce mémoire, nous avons évalué l'introduction de l'anamnèse médicamenteuse, faite en hôpital par les pharmaciens cliniciens, en officine ouverte au public.L'analyse de cette anamnèse a été réalisée au moyen d'outils validés et présents dans la littérature. Le résultat a ensuite été synthétisé par la réalisation d'un schéma de médication remis au patient.Enfin, ces étapes nous ont permis de mettre en avant les forces et les faiblesses d'une telle approche en officine dans le but d'améliorer le traitement du patient en collaboration avec le médecin.
