Abstract

"People who are not content to remain the gender they were assigned at birth have existed throughout human history and in all recorded cultures (Herdt, 1996). Naturally the experiences of people in the contemporary high-income cultures which are the focus of this book will be informed by the understandings and technology available now, but fundamentally gender diversity is not a new phenomenon. It is therefore curious that, until recently, it was commonly thought that gender diversity was so rare that it would not trouble most psychiatrists or psychologists in their usual course of work. This is no longer the case, as there has recently been an exponential increase in the number of gender diverse people coming forward, which mirrors that of same-sex-attracted people a couple of decades before. Most likely this is once again due to the greater, although still tenuous, social acceptance being shown towards sexual and gender diversity in some countries. It follows therefore that psychiatrists and others will have seen gender diverse people throughout their careers, but without necessarily being aware that they were doing so"--

Abstract

"Many parents experience fear, shame, and loss upon learning that their child is same-sex oriented or gender non-conforming. Therapies to help parents become more accepting and foster meaningful relationships with their LGBTQ children are critical. Ongoing parental criticism, invalidation, and rejection of one's sexual or gender identity can take a profound psychological toll and lead to internalized homophobia, expectations for future gay-related rejection by others, depression, and other negative impacts. In contrast, parental acceptance is associated with lower rates of suicidal thoughts and suicide attempts, higher self-esteem, higher levels of perceived social support, lower levels of psychological symptoms, and better general health. While most parents become more accepting, or at least more tolerant, over time, others remain partially or fully rejecting even years after learning of their child's identity. Attachment-based family therapy for sexual and gender minority young adults (ABFT-SGM) helps reduce parental rejection, facilitate parental acceptance, and ultimately promote safer, closer, and more mutually respectful relationships between LGBTQ young adults and their parents. Abstract

This book aids clinicians in supporting and caring for transgender and gender-diverse children and adolescents – youth who are born into an incongruent body. A recent study using data from 19 states reported that 1.8% of American youth identified as transgender. Many people who are transgender will experience gender dysphoria, the intense emotional distress that is caused by a discrepancy between a person's gender identity and their sex assigned at birth. Abstract

There are approximately 1.4 million trans-identified individuals in the US alone, many of whom will undergo gender-affirming medical or surgical interventions to better align their appearance with their gender identity. Multiple major medical societies recommend fertility preservation counseling prior to starting any gender-affirming therapies, but data are limited on the reproductive effects of common gender-affirming hormone regimens. The burden of fertility counseling falls to the hormone providers and surgeons that are encountering these patients, many of whom will not have had adequate training or resources to provide evidence-based recommendations and options. Additionally, many reproductive health care providers are not trained in how to care for gender minorities. The purpose of this book is to be a reference for clinicians and researchers in the field of transgender medicine, to provide up-to-date data and resources to properly counsel transgender and nonbinary patients about the reproductive consequences of gender-affirming interventions and their options for family-building, and to educate providers about appropriate and culturally competent reproductive health care. Effects of masculinizing and feminizing hormone therapy, as well as the fertility preservation options available, are discussed in detail for both adults and youth. In addition to these medical considerations, both psychosocial, legal and ethical considerations are highlighted for a more well-rounded presentation. A final chapter describes how to create a welcome and accepting clinical environment. Such a reference does not currently exist, leading to the propagation of misinformation and encouraging patients to seek nonmedical sources, such as social media, for their information. Reproduction in Transgender and Nonbinary Individuals fills in this gap as a timely text for reproductive endocrinologists, surgeons and all clinical staff working with this population. .
