Du milieu du XVIIIe siècle à nos jours, la mortalité maternelle a été divisée par 131, et la mortalité infantile par 69. A présent, dans les sociétés développées, les enfants qui naissent et leurs mères sont assurés de vivre. L'enfant désiré est le point de rencontre de ce repli de la mort, déplacée vers la vieillesse, et d'une longue histoire de contention puis de réduction de la fécondité, un processus propre à l'Europe de l'Ouest, marqué par trois stades : le recul de l'âge au mariage ; la chute de la fécondité à l'intérieur du mariage (réponse au recul de la mortalité infanto-juvénile) ; la mise au point de techniques efficaces de prévention des naissances non désirées. Alors, la société n'a plus besoin du mariage comme opérateur de la réduction de la fécondité et il n'est plus nécessaire d'interdire les relations sexuelles précoces pour garantir la collectivité contre l'excès des naissances. C'est la logique de l'enfant désiré qui façonne l'individu moderne et organise sa psychologie. De "cellule de base" de la société, la famille devient la "cellule de base" de l'individu. Articulant les savoirs de nombreuses disciplines, cet ouvrage, qui rend intelligible ce qu'on appelle l'individualisation, en constitue, à tout point de vue, une nouvelle définition.
