The goal of this Special Issue is to disseminate emerging techniques and innovative solutions that comprehensively address unmet needs in cardiovascular disease and can be rapidly translated into the clinical arena in order to significantly improve diagnostic accuracy and precision in treatment delivery, as well as to enhance therapy guidance and procedural success. The volume includes research contributions from cross-disciplinary scientists and professionals who work in the cardiovascular field at the interface of basic and translational research, clinical practice, medical (bio)physics, engineering, mathematics, and computer science. Several compelling contributions are focused on the development of advanced techniques in cardiovascular imaging (MRI, CT, ultrasound, optics) to investigate structure-function interaction and identify pathology, image analysis (e.g. registration, segmentation, visualization), deep-learning/AI classification methods to better characterize tissue and physiological signals, novel preclinical experimental models and clinical approaches employed in electro-anatomical mapping and image-aided therapies (e.g., cardiac ablation, resynchronization), as well as innovative interventional procedures for vascular applications.

Advances in the understanding of cardiomyopathies and the growth of cardiology have paralleled one another. Over the years, many classification methods have been developed for cardiomyopathies based on aetiology, structural models and the functional approach. The new concepts and events that have occurred in cardiology generally and in cardiomyopathies specifically are sufficient changes to justify this book. Developments in cardiomyopathies have advanced at a rapid rate, largely because of the progress that has been made in instrumental analysis and the integration of clinical cardiology with other fields of basic research. The material in this book encompasses and blends the knowledge recently acquired in genetics, pathology and physiology with the practical matters of diagnosis and treatment.

Cardiovascular responses to physical and/ or mental stressors has been a topic of great interest for some time. For example, significant changes of cardiovascular control and reactivity have been highlighted as important mechanisms for the protective effect of exercise as a simple and effective, non medical therapy for many pathologies. However, despite the great number of studies performed to date (e.g. >54,000 entries in Pubmed for “cardiovascular stress”), important questions of the role stress has on cardiovascular function still remain. For instance, What factors account for the different cardiovascular responses between mental and physical stressors? How do these different components of the cardiovascular system interact during stress? Which cardiovascular responses to stress are the most important for identifying normal, depressed, and enhanced cardiovascular function? Can these stress-induced responses assist with patient diagnosis and prognosis? What impact does physical fitness have on the relationship between cardiovascular function and health? The current topic examined our current understanding of cardiovascular responses to stress and the significant role that physical fitness has on these responses for improved function and health. Manuscripts focusing on heart rate variability (HRV), heart rate recovery, and other novel cardiovascular assessments were especially encouraged.

The Aim of "Current Concepts of General Thoracic Surgery" is to provide a brief overview of several topics in this field. It includes a collection of contributions from many outstanding Authors who provide their knowledge and experience from many countries around the world. We apologize for the chapters reviewed that have were not chosen for publication in this book; however, according to the single centres experience, the final result offers thorough and precious information on the several topics evaluated by the Authors. The wide range of subjects discussed goes from CT assessment of solitary pulmonary and metastatic nodules to prospective studies of drug delivery in thoracic surgery including surgical risk prediction, stress reaction, robotic pulmonary and cardiac procedures, vascular and thoracic reconstruction techniques, thoracic trauma and mediastinal fistula. I believe that this book represents an enhancement in the knowledge and in the involvement of individuals dedicated to these areas of study. It is my duty and pleasure to thank colleagues who helped me in the interesting and stimulating review process; Dr. Stefano Pasquino for cardiac surgery and Professor Francesco Puma for his many worthwhile suggestions.
