This book is a collection of scholarly articles on the themes of accountability and responsibility in health care and seeks to be the premier book in that field. It includes selected papers from the 2009 Jerusalem Conference, analytic essays on how accountability and responsibility are playing out in eight different countries, and reprints of some of the classic articles in the field.The book will interest policymakers, managers, researchers and students, and many of the ideas presented here will help shape the development of this field inthe years ahead. Some of these ideas have appeared in other forums; the unique contribution of this volume is that it is the first to bring together so many different perspectives on accountability and responsibility in health care. Health is largely determined by factors outside the health care domain. This highly successful textbook is now available in its third edition. Over the years it has become the standard textbook in the field world-wide. It mirrors the huge expansion of the field of economic evaluation in health care, since the last edition was published in 1997.This new edition builds on the strengths of previous editions, being clearly written in a style accessible to a wide readership. Key methodological principles are outlined using a critical appraisal checklist that can be applied to any published study. The methodological features of the basic forms of analysis are then explained in more detail with special emphasis of the latest views on productivity costs, the characterisation of uncertainty and the concept of net benefit. Managing health systems has become extremely complex. Intricate and often conflicting cultural, scientific and economic characteristics lead to the need for more elaborate analysis and more efficient policy instruments.The evaluation of health systems' performance and of their reforms also requires a robust information infrastructure in each country. Cross-national comparability across the OECD area remains an objective. There is a fundamental contradiction at the core of health policy in the EU that makes it difficult to draw a line between EU and Member State responsibilities. This raises a number of difficult questions for policy makers and practitioners as they struggle to interpret both 'hard' and 'soft' laws at EU and Member State level and to reconcile tensions between economic and social imperatives in health care.
