Abstract

With upwards of 4.5 million deaths worldwide each year, and more than one tenth of these occurring in those with no previously documented heart disease, sudden arrhythmic death (SAD) is both a major public health burden and a highly emotive issue for society at large. Recent years have witnessed a marked expansion in our knowledge of the physiology underlying SAD, both in the context of hereditary and acquired cardiac disorders. Thanks largely to work in genetically modified animals, the growth in our understanding of mechanisms underlying arrhythmia in the hereditary channelopathies has been particularly marked. Our growing knowledge of the fundamental mechanisms underlying SAD has so far failed to spur substantial developments in clinical practice. Despite a large body of work in both humans and animals, it remains impossible to confidently identify those at high risk of SAD, making pre-emptive therapy a challenge. What is more, with the thankful exception of the implantable cardioverter-defibrillators and pharmacological agents in very specific situations, there has been depressingly little progress in finding new and effective therapies. This Research Topic aims to go some way towards bridging the gap between advances in basic science and the development and delivery of new therapies. It brings together original research contributions and review articles from key opinion leaders in the field, focusing on the direct clinical implications of the basic science research now and in the future.

This book considers the practical management of sudden death and offers first-hand reflections of how emergency physicians, nurses, and allied health professionals cope. Sudden death is one of the most difficult aspects of emergency care, and the traumatic nature of some deaths can be devastating for the family and the emergency team. This book shows how practitioners confront the sudden death and the essential steps taken to manage the event that may have a critical impact on the grieving relative. The book explores the unique interactions between emergency and allied health practitioners and nurses, those who grieve and, the body itself. By understanding what is involved in sudden death work, and the practical, psychosocial and spiritual tensions that arise from managing the event and sequel, it may be possible to provide a more responsive service. The book addresses sudden death from the multi-professional emergency and allied services perspective to guide either the seasoned practitioners or the new and apprehensive recruit. National emergency response systems have been strengthened to cope with the increasing trauma and disease burden which, for many unfortunate individuals, results in an immediate or swift death trajectory. This multidisciplinary teaching text uses first-hand detailed stories of sudden death encounters. Abstract

Williams examines TASER use and high-risk group theory, which posits that people with certain physiological attributes, such as heart disease, mental illness, or drug use, are at increased risk of sudden death following application of a TASER electronic control device (ECD). Data derived from autopsy reports indicate few differences in the presence of such attributes between arrest-related sudden deaths following the application of an ECD and arrest-related sudden deaths that did not involve an ECD. The notable exception was deaths involving excited delirium, which appeared statistically more often in ECD-proximate events.

Abstract

Crib death (SIDS) is the most frequent cause of death for infants during the first year, striking 1 out of every 700-1,000. Scarce knowledge in the field of SIDS and its pathology has led to a continued and growing concern with finding an explanation, with the goal of being able to either predict or quickly diagnose the infant or term fetus. A systematic study of the autonomic nervous system and cardiac system has been performed on a large number of infants and fetuses who died suddenly and unexpectedly, as well as in age-matched control cases. The neurological and cardiac findings are described here, and the relationship between SIDS and unexplained fetal death is discussed. This book helps pathologists, forensic pathologists, pediatricians, obstetrics, and neonatologists in recognizing all potential morphological substrata and puts forward a well-researched postmortem study for use in a standardized autopsy protocol for use in all cases of sudden unexpected infant and fetal death.

Abstract

Crib death or sudden infant death syndrome is surely among the worst emotional tragedies that any parent can experience. It is the most frequent death-causing syndrome during the first year of life, striking one infant in every 700-1,000. Crib death is a great enigma, and one of the main open issues in the social-medical and scientific setting of contemporary medicine. Despite a wide spectrum of theories and years of research, its etiology remains unclear. The scarcity of knowledge in the field of crib death and its pathology has led to continued and growing concern, prompting an expanding search for explanation that will permit either prediction or rapid diagnosis.   This book describes systematic studies of the cardiovascular system and autonomic nervous system carried out in a large number of infants, newborns, and fetuses who have died suddenly and unexpectedly, as well as in age-matched control cases. The cardiovascular and neuropathological findings are presented in detail, and the relationship between crib death and unexplained perinatal death is discussed. This monograph will aid pathologists, forensic pathologists, pediatricians, obstetricians, and neonatologists in recognizing all potential morphological substrata. It puts forward a well-researched standardized postmortem protocol to be applied in all cases of sudden unexpected infant and perinatal death.
