Au cours de mon quatrième doctorat, j’ai effectué un stage à la maison médicale du Vieux-Molenbeek, située dans un quartier défavorisé. J’ai été très impressionné par le nombre de patients souvent de maladie allergique. Ces personnes se plaignaient souvent de la mauvaise qualité de leur logement. J’ai pu aussi me rendre compte les difficultés très diverses auxquelles étaient confrontées le praticien dans la prise en charge de cette maladie. Cette complexité m’a interpellé et m’a amené à réaliser ce travail.
La maladie allergique, classée par l’OMS au sixième rang mondial des grandes pathologies, est un véritable problème de santé publique. Actuellement, les maladies allergiques progressent de manière inquiétante au point que l’on prévoit un doublement du nombre d’asthmatiques dans dix ans en Europe occidentale. Son coût ne cesse d’augmenter et cette situation préoccupe le monde politique belge. L’environnement joue un rôle indéniable dans le domaine des maladies allergiques. Et l’influence de « l’indoor pollution » sur les maladies allergiques est plus que jamais d’actualité.
L’objectif de ce travail est de sensibiliser le médecin à l’influence de l’environnement sur les maladies allergiques et d’optimaliser la prise en charge par le médecin généraliste de patients défavorisés souffrant de maladies allergiques. En premier lieu, il est très important de bien connaître les facteurs environnementaux favorisant le développement des maladies allergiques. La problématique du logement dans les quartiers défavorisés doit être évoquée dans ce travail car la qualité de l’air intérieure dépende en partie de l’infrastructure du bâtiment. Ensuite, j’essayerai de démontrer les inégalités qui existent entre les personnes favorisées et défavorisées face aux maladies allergiques car c’est en comprenant ces inégalités qu’on saura agir efficacement sur la maladie. Enfin, le plus important est d’offrir aux médecins un outil qui lui permettre d’apporter aux patients des solutions adaptées à leurs moyens. La prise en charge d’un patient allergique est donc très globale et il est important pour le praticien de comprendre tous les facteurs qui peuvent jouer sur l’évolution de la maladie.

Despite medicine being grounded in biology, evolutionary biology is a relatively disregarded field. Macroanatomical features of our species have changed very little in the last 300,000 years, but there have been subtle changes in metabolism and immunology, including adaptations to dietary innovations, and adaptations to exposure to novel pathogens. Rapid pathogen evolution and evolution of cancer cells cause major problems for the immune system to find adequate responses. Moreover, many adaptations to past ecologies have turned into risk factors for somatic disease and psychological disorder in our modern world (i.e. mismatch), among which epidemics of autoimmune diseases, cardiovascular diseases, diabetes, and obesity, as well as several forms of cancer stand out. In addition, depression, anxiety, and other psychiatric conditions add to the list.
