Abstract

Schizophrenic adults and psychotic children present many cognitive dysfunctions which have severe consequences in everyday life. The mechanisms underlying cognitive impairment are poorly known nowadays in term of severity, onset time, evolution, impact on everyday life and therapeutic interest. Understanding these mechanisms could help making early detection of the disease and improve the care brought to patients for a possible cure. The present work aims to contribute to the disease understanding by looking for links between the cognition of adult schizoph renic and psychotic children.Three children, Marie, Simon and Sarah, were examined usmg the Wechsler scale and cognitive aspects were compared with data from the literat ure on cognition in schizophrenic adults. Disorders were highlighted in both children and adults regard ing memory, attention and concentration, executive fonctions, motor fonction, verbal fluency and IQ. These disorders affect heterogeneously ail patients and at varying degrees .The two dominant assumptions in recen t literature, neurodevelopmental and neurodegenerative , regard ing the etiology of the disease are discussed in the light of the 1inks observed, in this work, between cognition of schizophrenic adult and psychotic children.In addition, two approaches have been advanced regarding the origin of cognitive impairment in psychosis: the first one stands the cognitive impairments at the onset of psychotic symptoms while the second one describes the debilitating cognitive impairment as a consequence of the onset of psychotic symptoms.Finally, as an improvement of the cognitive impairment can be expected, the therapeutic side is covered through the recent literature. Les adultes schizophrènes et les enfants psychotiques souffrent d'une altération de la cognition qui engendre des conséquences au quotidien. Les mécanismes à l'origine des troubles cognitifs sont à ce jour mal connus : sévérité, moment d'apparition, évolution, impact sur la vie quotidienne, intérêt thérapeutique,... Leur meilleure compréhension pourrait permettre de rendre précoce la détection de la maladie et d'améliorer la prise en charge des patients en vue d'une éventuelle guérison. Ce présent travail vise à apporter une contribution à la compréhension de ce type de pathologie par la recherche de liens entre les troubles cognitifs rencontrés chez'les adultes schizophrènes et les enfants psychotiques.Trois enfants, Marie, Simon et Sarah ont été examinés grâce aux tests de Wechsler et leurs aspects cognitifs ont été comparés à ceux des adultes via les données de la littérature scien tifique récente. Chez les adultes et les enfants, des troubles similaires mais aussi diffërents ont été mis en évidence au niveau de la mémoire, de l'attention et la concentration, des fonctions exécutives, des fonctions motrices, de l a fluence verbale et du quotient intellectuel. Ces troubles atteignent l 'ensemble des patients de manière très hétérogène .Les deux hypothèses dominant la littérature actuelle, neurodéveloppementale et neurodégénérative, concernant l'étiologie de la maladie sont discutées à la lumière des liens observés, dans ce travail de fin d'étude, entre le fonctionnement cognitif des adultes schizophrènes et des enfants psychotiques.De plus, deux approches sont avancées concernant l'origine des troubles cognitifs dans la psychose : la première place les troubles cognitifs à l 'origine de la symptomatologie psychotique alors que la seconde explique l'expression invalidante des troubles cognitifs comme une conséquence de l'apparition de la symptomatologie.Enfin, une amélioration des troubles cognitifs pouvant être espérée, le versant thérapeutique est parcouru à travers la littérature récente.

Abstract

L'alexithymie est une déficience dans l'identification et l'expression des émotions. Nous savons que cette caractéristique a une implication sur certaines maladies physiques ou psychiques. Dès lors, nous avons voulu étudier l'impact de celle-ci sur une maladie chronique, à savoir le diabète (types 1 et 2 confondus). Nous avons ainsi exploré l'alexithymie et ses im plications sur le diabète, ainsi que ses relations avec la dépression, le lieu de contrôle et les caractéristiques sociodémographiques du sujet.Méthodes :40 patients diabétiques et 173 contrôles ont été inclus dans cette étude. L'alexithymie a été évaluée par le « Toronto Alexithymia Scale » (TAS-20) et Je « Bermond-Vorst Alexithymia Question naire » (BVAQ), la dépression par l'inventaire de Dépression de Beck (BDI-13), le lieu de contrôle par le Questionnaire du lieu de contrôle multidimensionnel de la santé.Résultats :Les sujets diabétiques étaient plus alexithymiques que les sujets contrôles (F = 4,163 ; sig = 0.043). La relation entre alexithymie et dépression a été observée tant chez les sujets contrôles que chez les sujets diabétiques (R = 0.433 ; sig < 0.001). Les sujets diabétiques étaient plus dépressifs que les sujets con trôles (F = 8.640 ; sig. = 0.004), mais cette relation disparait si nous contrôlons l'alexithymie. Nous avons également mis en évidence que les sujets diabétiques étaient plus externes que les contrôles, avec un Pouvoir des autres plus important (PHLC). Au sein de la population contrôle, les sujets en couples étant moins alexithymiques que les sujets non en couple (F = 5.591; sig. = 0.019) ; l'alexithymie était décroissante en fonction du niveau d'études (F = 3.628 ; sig. = 0.014).Conclusion : Notre recherche confirme que l'alexithymie est un facteur défavorable de la santé. L'origi nalité de notre travail est de discuter le lien entre l'alexithymie et la gestion du diabète. En vertu d'une approche bio-psycho -sociale de la personne et de sa maladie, une gestion efficace du diabète voudrait que le soignant tienne compte de l'alexithymie, de la dépression, du lieu de contrôle mais aussi de la vie relationnelle et affective de son patient. We have explored alexithymia and its implications for diabetes and its relationship with depression, locus of control and sociod emographic characteristics of the subject.Methods : 40 diabetic patients and 173 controls were included in this study. Alexithymia was assessed by the "Toronto Alexithymia Scale" (TAS-20) and the "Bermond-Vorst Alexithymia Questionnaire" (BVAQ), depression by Beck Depression Inventory (BDI-13), locus of control by the Question naire of the place of multidimensional health monitoring.Results :Diabetics were more alexithymic than controls (F = 4.163; sig = 0.043). The relationship between alexithymia and depression has been observed both in controls than in diabetic subjects (R = 0.433; sig <0.001). Diabetic subjects were more depressed than controls (F = 8.640; Sig = 0.004), but this relationship disappears if we control alexithymia. We have also shown that diabetics are more external than controls, with a Power of other larger (PHLC).Within the population, control subjects in couple are less alexithymic than non-in-cou plesubjects (F = 5.591;. Sig = 0.019); alexithymia was decreasing with the Jevel of education (F = 3.628; sig = 0.014).Conclusion : Our research confirms that alexithymia is an unfavorable factor for health. The originality of our work is to discuss the relationship between alexithymia and management of diabetes. Under a bio-psycho-social approach of the person and of his disease, effective diabetes management wants the caregivers to take account of alexithymia, depression, locus of control but also the social and emotional life of his patient.
