Abstract

The health outcomes of millions of individuals with cardiovascular risk factors, who are vulnerable to strokes and heart attacks, can only be improved through an integrated approach that addresses overall cardiovascular risk. There are many barriers to implementing such an approach, particularly in low-resource settings. The WHO CVD-Risk Management package has been developed in order to facilitate integrated care of cardiovascular risk despite these barriers. It informs policy-makers of the need and feasibility of managing cardiovascular risk in less well-resourced settings and enables cardiovascular risk management (in settings faced with low resources) through affordable approaches and rational resource allocation. It also facilitates a paradigm shift from single risk factor management to comprehensive cardiovascular risk management through simplified clinical pathways and promotes evidence-based non-pharmacological treatment of cardiovascular risk and operationalization of clinical prevention aided by counselling protocols. Finally, it promotes the use of cost-effective generic drugs for management of cardiovascular risk and empowers patients and their families to cope with a long-term illness.

Abstract

Transcranial magnetic stimulation (TMS) is used both as a diagnostic instrument and for therapy, available only at some psychiatric clinics for treatment of depression and at clinical neurophysiology where TMS is used for diagnosis of nerve damage. The Swedish National Board of Health and Welfare issued a referral edition about the use of repetitive TMS as an alternative treatment for depression. This may lead to a major increase in the application of TMS to treat depression. TMS is based on induction of an electric (E) field inside the brain by application of an external magnetic field with rapid rise and fall time. The E field in the brain has been calculated when different coils were used for the treatment. The reported E fields are of the order of tens to hundreds of volts per meter and the induced current density is estimated at tens of A/m2. This field can depolarize neurons or modulate cortical excitability by selecting the appropriate parameters for stimulation and the duration of the treatment session. The mechanisms of action of neurostimulation still remain incompletely understood.

Selected by Choice as one of the outstanding publications for 1991. Are risk debates disputes between those who accept the findings of science and those who do not? Between good and bad science? Or is it possible that opposing assessments of risk, by scientific experts as well as ordinary citizens, reflect and are guided by dominant values held by the assessors? The following analysis of one of these debates supports the latter view. In it we suggest what those dominant values are, how they work within a risk assessment, and some implications of reconceiving risk debates as primarily debates about values.

Will SARS or another pandemic influenza reoccur and, if it does, have we learned how to manage pandemics more effectively? In SARS Unmasked risk communication expert Michael Tyshenko offers answers to this and other questions. Cathy Paterson, who worked as a nurse clinician during the Toronto SARS crisis, adds an important view from the frontlines. Their analysis reveals an out-of-control situation with mixed risk communication messages, a lack of leadership, and an overwhelmed health care system that was unable to both cope with the crisis in Toronto and provide adequate support for their most valuable employees at the time - health care workers.
