Historically, the doctor-patient relationship has been based on a one-way trust—despite recent judicial attempts to give patients a greater voice through the doctrine of informed consent. Cet ouvrage, basé sur les récits de dilemmes éthiques vécus en contexte, a pour objet de repenser les conditions locales de la prévention, trop souvent ignorées par les régulations internationales.Les récits et réflexions de médecins d'Afrique francophone présentent un quotidien de dilemmes liés non seulement à la rareté des médicaments mais aussi à la représentation de leurs effets sociaux. Leur confrontation met en scène l'inadéquation des politiques de prévention, diffusées par les organismes régulant la recherche. Ils nous invitent à repenser, à travers la parole donnée aux acteurs de terrain une régulation éthique de la recherche plus adaptée à leur vécu.L'alternative proposée est basée sur l'idée de capabilité, que nous devons à Amartya Sen. Ce scénario vise à développer un modèle d'éthique globale qui conditionne la légitimité de la recherche dans les pays en voie de développement à l'accès aux soins pour les populations locales.
