Even the best hernia repair can result in postoperative difficulties for the patient caused by repair sequelae as for example pain, infertility, infection, adhesion and dislocation of the protheses. That can happen many years later and now, where the general principle of hernia repair is well understood all over the world, these sequelae are noticed more and more. To define them, to evaluate the absolute and relative risk of these sequelaes and to describe the ways of their prevention, diagnosis and treatment, the 5th Suvretta meeting had focussed on this subject. We discussed if there’s a principle risk by technique, material or both. The results of these discussions and the future handling and evaluation of this problem was the aim of this meeting. Even the best method can be made better by optimization of its single components. This can happen many years later, and now that the general principle of hernia repair is broadly understood all over the world, these sequelae are being noticed more and more. The 5th Suvretta meeting was held in order to define these sequelae, to evaluate the absolute and relative risks they pose, and to discuss the methods of their prevention, diagnosis and treatment. We discussed whether the principal risk was related to technique, material or both. This discussion and the future approach to and evaluation of this problem were the aims of the meeting, working on the premise that even the best method can be made better by optimizing its individual components.

Incisional hernia surgery has witnessed an important progress over the last years, not only as far as the pathophysiological and etiopathogenetic aspects are concerned, but also from a technical point of view. Thanks to the recent findings on genetical disorders of the collagen, incisional hernia is today considered a true "disease of the abdominal wall". Surgeons need to be aware of all the different repairing techniques, in order to make an elective choice and to keep side effects and recidives under control. Today, on the basis of the most recent research on synthesis of collagen and collagenolysis, the treatment of the incisional hernia should not disregard the use of synthetic prostheses and of biomaterials, both in the routinal surgical practice and in the most complex clinical cases. Both informative and easy to read Transanal stapling techniques for anorectal prolapse provides a comprehensive review of the current knowledge regarding the modern treatment of anorectal prolapse, with emphasis on stapled haemorrhoidopexy for prolapsing haemorrhoids, and the STARR procedure for obstructed defaecation syndrome. The first transanal stapling technique for the treatment of anorectal prolapse was presented in abstract form in 1998 by Dr A Longo. This described a novel technique for the surgical correction of prolapsing hemorrhoids. Dural cavernous sinus fistulas (DCSFs) represent a benign vascular disease, consisting in an arteriovenous shunt at the cavernous sinus. In the absence of spontaneous resolution, the fistula may lead to eye redness, swelling, proptosis, chemosis, ophthalmoplegia and visual loss. Although modern imaging techniques have improved the diagnostic, patients with low-flow DCSFs are still misdiagnosed. These patients can get erroneously treated for infections and inflammation for months or years and are at risk of visual loss. Early and proper diagnosis helps to avoid deleterious clinical course of the disease. This volume provides a complete guide to clinical and radiological diagnosis as well as to therapeutic management of DCSF with emphasis on modern minimal invasive treatment options. It commences with an informative description of relevant anatomy. After sections on the classification, etiology and pathogenesis of DCSF, the clinical symptomatology of the disease is described in detail. The role of modern non-invasive imaging tools is then addressed with the use of computed tomography, magnetic resonance imaging and ultrasound.
