Abstract

Pancreatic cancer is the deadliest of the gastrointestinal tract with 5-year survival rates of less than 5%. Given common asymptomatic early disease course, most patients (50%) present with an already metastatic disease, while only 20% can undergo potentially curative resection. The remaining 30% present with locally advanced disease, defined as extended vascular encasement, where the risk of surgical therapy often outweighs its benefits. Traditional thermal local ablative modalities (RFA, MWA, or cryotherapy) have the disadvantage that they are not applicable in proximity to vital vascular structures, which are abundant in the peripancreatic region. Irreversible electroporation (IRE) is an emerging non-thermal alternative that induces apoptosis of tumor cells by the delivery of short repetitive impulses of high-voltage electric current. Given its mostly non-thermal modality, IRE is not hampered by a heat-sink effect and is applicable with little risk around vascular structures, bile and pancreatic ducts. Recent research suggests that local tumor destruction through IRE improves overall survival, progression-free survival and quality of life in patients with locally advanced pancreatic cancer.

Abstract

'Pancreatology: from Bench to Bedside' focusses on recent advances in pancreatic basic research, genetics of the pancreatic diseases and its surgical management. Basic research discusses the implication of pancreatic stress protein in acute pancreatitis and pancreatic cancer and their possible role as therapeutic targets. Also, very original results show the unexpected role of lipids as mediators during acute pancreatitis. Gene screening strategies allow the detection of the genes responsbile for gemcitabine resistance of pancreatic cancer cells . They lead to the selection of several target genes, in order to suppress the resistance of cells to gemcitabine treatment. The mechanism by which tetrahydrocannabinol is anti-tumoral in pancreatic cancer cells is presented and the use of THC as a promising new therapeutic agent is discussed. Genetic data are shown concerning hundreds of families with hereditary chronic pancreatitis and their possible role in the pathogenesis of the disease. Another very original study addresses the prevention and treatment of pancreatic diseases with diat. In clinical research, convincing data about the use of endoscopic sphicterotomy in the management of acute bilary pancreatitis is presented, based on the experience of a Center highly specialized in pancreatic diseases.

Abstract

Pancreatic diseases are a crucial issue not only because of their epidemiological significance, and their anatomical and metabolic complexity, but also because of the different treatment options available and their implications for surgery. Due to the slow but steady increase in the incidence of pancreatitis and pancreatic cancer, these diseases are today a real challenge for the surgeon.
