Abstract

Auf fahrende Züge aufzuspringen, zählt nicht zu den charaktervollsten Formen der Fortbewegung. Und dennoch gibt es Angebote, die man nicht ausschlagen kann, weil sie einfach zu verlockend sind. Die Nachfolge von Felix Harder bei diesem Standardwerk anzutreten, war gleichermaßen ehrenvoll und verpflichtend. Standards, die Felix Harder zusammen mit den beiden anderen Herausgebern gesetzt hatte, galt es zu halten, Gutes sollte bewahrt, Bewährtes gegebenenfalls verbessert werden. Ergänzungen waren gewünscht (so die komplette Hernienchirurgie), aber der Umfang des Bandes durfte nicht - nehmen, wohl aber die Qualität eines ohnehin schon sehr guten Werkes. Diese Aufgabe war verlockend, aber eigentlich kaum lösbar, und nun liegt der fertige Band vor uns. Dass dies möglich war, verdanke ich vor allem den bisherigen Autoren, die ohne Murren ihre Beiträge großzügig auf das Essenzielle gekürzt und auf den aktuellen Kenntnisstand ausgerichtet haben. Aber auch allen neuen Autoren ist zu danken, dass sie sich in dieses Konzept eingebunden und uns mit ihren Ausführungen bereichert haben. Insbesondere danke ich meinen Mitarbeitern und hier vor allem in erster Linie Herrn Kollegen Priv. -Doz. Dr. Marc Jansen für die straffe Hand bei der Realisation in de- bar kurzer Zeit. Ohne das jetzige und frühere Aachener Team wäre diese Aufgabe nicht zu lösen ge- sen, hierfür danke ich allen Beteiligten. Abstract

Neurotraumatology patients present an enormous challenge to society. Neuros- gical management of brain and spinal cord injury has been a frustrating area, as s- gical methods, especially in moderate and severe injuries, have been limited to control of brain and spinal compression, control of intracranial pressure with its expected effect on cerebral blood flow, and structural repair of the supporting structures (skull, spine, brain and spinal cord coverings). Achieving the best outcome for the neu- traumatology patient, however, requires much more than that. This important issue has thus far been approached through the broadest spectrum of scientific knowledge, from the most fundamental biological sciences to the social sciences. Such a variety of approaches has for a long time been in need of a "center of gravity;' a balancing point where facts and opinions can meet and be integrated. The management of neural injury is awaiting a breakthrough, and we should do our best to facilitate. As the majority of the new discoveries tend to appear at the integrating borderlines of two separate scientific areas, our duty is to bring together all scientists involved with neural injury. all hope that the meeting has fulfilled this goal and is a step forward in bringing together previously distant areas of knowledge in neurotraumatology. These proceedings contain the full text of the submitted reports, with the intention of providing the information and personal opinions of the participating speakers to those who were unable to attend.

Abstract

Endoscopic access to the small intestine has for a long time presented a challenge to physicians. Although capsule endoscopy can provide an endoscopic image from the entire small intestine, neither biopsy nor endoscopic treatment can be performed with it. In order to make more definitive diagnoses of abnormal findings detected by capsule endoscopy, a new system incorporating greater insertability and maneuverability is required. Double-balloon endoscopy, which enables endoscopic observation of the entire small intestine, represents such a new system. The chapters of this book describe the principles, insertion technique, and clinical applications of double-balloon endoscopy, which will become the gold standard of enteroscopy and may well eliminate the need for intraoperative enteroscopy in many patients. As such, this book will provide an invaluable source of reference for all practitioners working in the field.

Abstract

In the age of the World Wide Web, informed patients continue to surprise oncologists with detailed questions about popular tumor therapies. Although minimally invasive tumor therapies (MITT) have become daily clinical practice for palliative treatment of liver tumors, the acceptance of these palliative modalities still varies enormously. In addition, promising results have been published for in situ ablation of lung metastases, bone tumors, and renal cell carcinoma. This book gives an up-to-date overview of the popular techniques and clinical results of MITT, with a clarification of the actual indications including the size, tumor entities, and clinical benefits. Abstract

Gastroesophageal reflux disease (GERD) is one of the most common disorders with an increasing prevalence and incidence in the last two decades. This book, edited by two experienced surgeons and a clinical psychologist in cooperation with numerous worldwide leading experts, presents clinically relevant information for gastroenterologists, internists, surgeons, residents and also nurses, who frequently care for GERD patients. Focusing on different treatment concepts – medical, endoscopic as well as surgical – the chapters include the basics of symptomatology and epidemiology, pathophysiology, GERD among different age groups, complications and its treatment, hiatal hernia or H. pylori and GERD, NERD and functional heartburn, diagnostic procedures and also presurgical examination.
