Abstract

A partir d'une analyse de la perception courante du corps gravement handicapé, de l'étude des exhibitions des monstres au XIXe siècle et de l'histoire de la tératologie scientifique, l'auteur s'efforce de comprendre ce qui se produit en nous face au corps difforme d'un individu et nous pousse à penser que c'est un « monstre ». En nous confrontant aux limites de notre tolérance, la grande difformité physique nous révèle nos craintes liées au corps mutilé, dégradé, non viable. Elle suscite des peurs irrationnelles de contamination et des fantasmes de métamorphose, mais aussi des angoisses rationnelles en touchant à la fragilité de l'organisme et au vécu intérieur du corps.

Abstract

The greatest dif?culty in life is to make knowledge effective, to convert it into practical wisdom. Sir William Osler. The inborn errors of metabolism, as a group of metabolic diseases, are re- tively rare and are sometimes called “orphan diseases. ” As a group, they account for about 1 in 2,500 births (Applegarth et al. 2000) and, as a cumulative group reaching 20 years of age, their prevalence is about 40 cases per 100,000 popu- tion. In terms of patient days of continuous supervision and care, hundreds of thousands of such days are involved per generation of these patients. Although experience with these diseases as a class may be small and people expert in their management may be relatively few, in the years to come many caregivers will become involved. This book offers help to them. Until the mid-twentieth century, hereditary metabolic and other genetic diseases were considered to be purely “genetic” problems. Destiny would take its course, treatment did not exist, and genetic counseling about recurrence risks was virtually all that could be offered. Phenylketonuria (PKU) was then shown to be a treatable genetic disease in which early diagnosis and effective treatment prevented the disease (mental retardation) in PKU. Other genetic diseases for which an environmental experience was an essential component of cause (e. g. , exposure to a dietary component or a drug) were then seen to yield to treatment.

Abstract

Are new reproductive and genetic technologies racing ahead of a society that is unable to establish limits to their use? Have the "new genetics" outpaced our ability to control their future applications? This book examines the case of preimplantation genetic diagnosis (PGD), the procedure used to prevent serious genetic disease by embryo selection, and the so-called "designer baby" method. Using detailed empirical evidence, the authors show that far from being a runaway technology, the regulation of PGD over the past fifteen years provides an example of precaution and restraint, as well as continual adaptation to changing social circumstances. Through interviews, media and policy analysis, and participant observation at two PGD centers in the United Kingdom, Born and Made provides an in-depth sociological examination of the competing moral obligations that define the experience of PGD. Among the many novel findings of this pathbreaking ethnography of reproductive biomedicine is the prominence of uncertainty and ambivalence among PGD patients and professionals--a finding characteristic of the emerging "biosociety," in which scientific progress is inherently paradoxical and contradictory. In contrast to much of the speculative futurology that defines this field, Born and Made provides a timely and revealing case study of the on-the-ground decision-making that shapes technological assistance to human heredity.
