L’anafilassi è definita come una reazione immediata sistemica dovuta al rapido rilascio IgE mediato di potenti mediatori dai mastociti tissutali e dai basofili del sangue periferico e rappresenta una condizione clinica generalizzata che include segni e sintomi cutanei, respiratori, cardiovascolari e gastrointestinali. Comprende svariati quadri clinici, che derivano dall’associarsi di sintomi come orticaria, angioedema, vomito, diarrea, asma, dispnea, edema della glottide, ipotensione, shock, ecc, non sempre di facile identificazione, che richiedono tuttavia un approccio diagnostico tempestivo e corretto. L’esigenza di chiarire i concetti attuali dell’atopia, che per troppo tempo hanno oscillato tra una primitiva visione squisitamente clinica ed una successiva unicamente immunologica, era matura da tempo. L’avere compreso che il ruolo delle immunoglobuline E non è esaustivo ed avere accettato la salomonica divisione tra atopia intrinseca ed estrinseca, sono stati i primi due passi fondamentali. Le recenti scoperte del ruolo dell’immunità innata, e quindi dei peptidi antimicrobici, hanno aperto dei fondamentali spazi di comprensione nell’atopia e non solo. Quello che ora appare chiaro è che l’atopia non è la malattia di un organo ma piuttosto un difetto delle membrane che delimitano il nostro "self". Il fatto che queste membrane si chiamino pelle, intestino, congiuntiva, bronchi o quant’altro ha artificialmente frammentato la comprensione ed il trattamento di un fenomeno unitario. Questo libro vuole cercare di fare una sintesi delle diverse esperienze ma soprattutto essere uno stimolo a far sì che la medicina si focalizzi costruttivamente sullo stesso bersaglio. Se così sarà, non assisteremo più a"guerre di religione" tra scienziati e medici pratici o, peggio ancora, tra i differenti specialisti che si contendono il paziente. Se l’atopia, come noi crediamo, è un problema globale, esso può essere adeguatamente affrontato solo in un contesto pluridisciplinare e in clima collaborativo, non solo tra medico e medico ma anche tra medico e paziente.

The goal of the International Immune-Mediated Diseases: From Theory to Therapy (IMD) Congress is to bring the best world immunologists and clinicians to Moscow to participate in plenary sessions, symposia and educational workshops designed to expose basic and clinical immunologist as well as practicing clinicians to the newest developments in the field. First IMD Congress consisted of 8 Plenary Sessions, 40 Symposia and 25 Workshops. The meeting attracted almost 2000 participants and speakers from 15 countries. After this great success of the First IMD Congress, the Organizing Committee decided to convene the meeting every other year and the Second International IMD Congress was planned for September 2007. Special attention will be given to innovative therapeutic modalities, allergy and asthma, immunodeficiency, tumor and transplant immunology, immunotherapy and vaccines, HIV and infectious diseases, autoimmune diseases, inflammation, immunomonitoring and immunodiagnostics. The idea for this book was born during the symposium on biologics organized by ZAFES (Center for Drug Research, Development and Safety at the University of Frankfurt am Main) in September 2005. Highly distinguished researchers special- ing in the field of biologics had gathered together to exchange information on this relatively new subject. Realizing that this symposium was one of the few sources of condensed information on biologics, it became obvious that we had to create a means of informing an interested wider circle of scientists and especially general c- nicians.
