Empathie is etymologisch afkomstig van het Griekse ?en? en ?patheia? en betekent aangedaan zijn door wat er in een ander gebeurt. Het is een complex concept, zowel verwant met als verschillend van andere psychoanalytische concepten zoals identificatie, concordante tegenoverdracht, attunement, containment, sharing, rapport, steun. Empathie verwijst naar het vermogen zich in de plaats te stellen van de ander, empathie is meeleven met en voelen wat de ander voelt. Empathie is een fenomenologisch en een psychotherapeutisch concept maar het blijft de vraag of empathie verzoenbaar is met analytische neutraliteit. Vaak in het begin van hun carrière gaan psychotherapeuten er ten onrechte vanuit dat empathie de garantie betekent voor therapeutisch succes en dat dit de noodzakelijke en voldoende voorwaarde vormt voor het welslagen van een therapie. Empathie is in de eerste plaats een intermenselijke vaardigheid die zich in de loop van het leven progressief ontwikkelt. Op basis van meta-analyses van outcome-onderzoek blijken specifieke behandelvormen en technieken slechts maximaal vijftien procent van de outcome variantie te verklaren. Zij liggen hiermee in een nek-aan-nek race met de placebo-effecten ?verwachting, hoop en geloof in verandering?. Veertig procent is toe te schrijven aan factoren buiten de therapie zoals spontaan herstel, toevallige factoren in het leven van de cliënt en sociale ondersteuning. De overige dertig procent tenslotte is toe te schrijven aan ?gemeenschappelijke factoren?, dit zijn relatiefactoren die in de meeste therapieën een rol spelen, zoals: empathie, betrokkenheid, aanvaarding van de cliënt, eigenschappen van cliënt en therapeut en van de therapeutische alliantie. Empirisch onderzoek van de jaren 1950 tot nu heeft aangetoond dat empathie tussen zeven en tien procent van de outcome variantie verklaart: minstens evenveel en waarschijnlijk meer dan het percentage dat verklaard wordt door specifieke interventies. In het boek wordt de betekenis van het concept ?empathie? voor de psychotherapie genuanceerd belicht. Er gaat veel aandacht naar het belang van empathie in psychoanalytische psychotherapie. Het geheel wordt geïllustreerd met klinische gevallen. Ook de neurobiologische aspecten en de ontwikkelingsgeschiedenis van empathie worden beschreven.
