Abstract

This thesis describes a detailed and critical analysis of a clinical case encountered in a pharmacy. The case chosen is that of ADHD – Attention Deficit Disorder with or without Hyperactivity – because of its increasing occurrence. Two patients with this disorder were analyzed side by side. Ina first part, the pathology and its treatment ar presented ona comprehensive manner. The treatment of ADHD is multidisciplinary: speech therapy, consulting a neuropsychologist, and with a child-psychiatrist and medication are often necessary. Methylphenidate (Ritaline®, Ritaline MR®, Concerta®) and atomoxetine (Strattera®) are the two key molecules for treating ADHD. Methylphenidate exists as different galenic forms. Some of them provide a longer effect (SODAS for Ritaline MR®, OROS for Concerta®) and therefore allow for a once-a-day administration. Their major drawback is their high price, as it also the case for Strattera®. (Ritaline® is the only form to be reimbursed when starting the treatment). In a second time, the study of the two clinical cases is presented, examining the treatment of both patients and describing interviews with the patients, but also their parents, teachers and doctors.
This thesis has allowed me to understand the essential role a pharmacist may play in giving advice to the patients and their care-givers on the treatment itself and its follow-up, as well as on non-pharmacological approaches that may help in improving the quality of life for these patients, and in helping parents and teachers to better understand the pathology or the treatment of the child Le sujet de ce mémoire est l’analyse critique et détaillée d’un cas rencontré en officine. Le cas choisi est celui du TDA/H - trouble déficitaire de l’attention avec ou sans hyperactivité - en raison de sa fréquence de plus en plus importante. Deux patients atteints par ce trouble ont été analysés en parallèle. Dans un premier temps, la pathologie, se prise en charge globale et pharmacologique sont développées. Le traitement du TDA-H est multidisciplinaire : rééducation logopédique, séances chez le neuropsychologue, le pédopsychiatre et un traitement médicamenteux sont souvent nécessaires. Le méthylphénidate (Rilatine®, Rilatine MR®, Concerta®) et l’atomoxétine (Strattera®) sont les deux principales molécules traitant le TDA/H. Le méthylphénidate est présent sous différentes formes galéniques. Certaines formes permettent d’avoir une durée d’action prolongée (SODAS pour la Rilatine MR®, OROS pour le Concerta®) et d !s lors de réduire la posologie à une prise unique/ l’inconvénient majeur de ces forme est leur prix élevé. Il en est de même pour le Strattera®. En effet, il n’y a que la Rilatine® simple qui est remboursée en première intention. Dans un deuxième temps, l’étude des deux cas cliniques est présentée. Elle s’appuie sur l’analyse du traitement des deux patients, mais aussi sur une rencontre avec les patients, leurs parents, enseignants et médecins. Ce travail m’a permis d’appréhender le rôle essentiel du pharmacien en ce qui concerne les conseils relatifs au traitement lui-même et à sn suivi, mais aussi les conseils non-pharmacologiques à prodiguer au patient et à son entourage

Abstract

ADHD is the most commonly diagnosed childhood behavioral disorder. Moreover, many students receiving special education assistance are reported to have ADHD. Children with ADHD may experience significant school adjustment difficulties and achievement problems. Problem behaviors associated with attention deficits and hyperactivity often have a negative impact on the classroom, and, therefore, may compromise the learning environment for many, if not all, students. There is a critical need for school professionals to identify, assess, and treat students with ADHD. Identifying, Assessing, and Treating ADHD at School brings science to practice, providing school professionals invaluable information to meet the needs of children with ADHD.
