Abstract

OBJECTIFS : Prise en charge optimale des infections urinaires (lU) chez la dame de plus de 65 ans. METHODE : Lecture de plusieurs revues de littérature et confrontations avec plusieurs spécialistes afin d’établir des recommandations stratégiques applicables en médecine générale et plus particulièrement chez la femme âgée de plus de 65 ans. RESULTATS : Avant tout, il semble indispensable de pouvoir déterminer à quel type d’infection urinaire nous devons faire face. En fonction de cette classification, dépendra non seulement la mise au point mais également le traitement et le suivi. CONCLUSION : Il n’existe pas, à l’heure actuelle, de guidelines bien établis en cas d’infection urinaire chez la personne âgée. L’évaluation au cas par cas reste, en effet, souvent nécessaire. Cependant, en fonction du type d’infection urinaire, plusieurs lignes de conduites sont applicables et à conseiller en médecine générale.

Abstract

L’uretere è un organo deputato unicamente al collegamento tra il rene e la vescica. Per la delicatezza della sua struttura anatomica e per la sua estensione in lunghezza che ne condiziona complessi rapporti anatomici con molti altri organi, detiene una fragilità intrinseca alla patologia propria, offre scarsa difesa al coinvolgimento da parte delle patologie di organi vicini ed è predisponibile ai traumi chirurgici ed endoscopici. Nella pratica clinica, queste caratteristiche vengono non infrequentemente sottovalutate, esponendo l’organo a pericolose sequele. Inoltre, lo sviluppo e la diffusione delle tecniche endourologiche, se da un lato rappresentano un brillante esempio di chirurgia mininvasiva, dall’altro hanno aumentato le possibilità di danno iatrogeno in corso o successivo alle manovre stesse. La monografia, dopo gli indispensabili capitoli sull’embriologia, l’anatomia, la fisiologia e la fisiopatologia dell’organo, offre la trattazione sistematica della patologia ureterale, per la prima volta affrontata “dal punto di vista dell’uretere”. L’intento è quello di evidenziare il suo ruolo diretto nelle malattie di apparato e di far risaltare l’importanza della rilevazione sintomatologica nei casi di coinvolgimento - piuttosto frequente - da parte di patologie viciniori o come conseguenza di svariate procedure chirurgiche. La metodica della introduzione di tricks and tips tende a evidenziare le criticità e a offrire suggerimenti clinici per prevenirle o superarle.
