Abstract

Over the past two decades it has been increasingly recognized that whole-body ultrasound is an invaluable tool in the critically ill. In addition to offering rapid whole-body assessment, it has the advantage of being a bedside approach that is available at all times and can be repeated at will. Accordingly, it permits the immediate institution of appropriate therapeutic management. Whole-Body Ultrasound in the Critically Ill is the sequel to the author¹s previous books on the subject, which were first published in French in 1992 and 2002 and in English in 2004. This new volume reflects the latest state of knowledge by including a variety of improvements, revised definitions, and updated algorithms. Findings in respect of individual organs are clearly presented, and a particular feature is the in-depth coverage of the lungs, traditionally regarded as an area unsuitable for ultrasound. Throughout, the emphasis is on the practical therapeutic impact of the technique. Its value in a variety of settings, including unexplained shock, management of hemodynamic instability, acute respiratory failure (the BLUE protocol), and the critically ill neonate, is carefully explained. Interventional ultrasound and less widely recognized applications, such as mesenteric infarction, pneumoperitoneum, and intracranial hypertension, are also described. Pitfalls of the technique receive due attention. Today, whole-body ultrasound touches upon every area of critical care. This book, from the chief pioneer in the field, shows that the technique enables critical care physicians to detect therapeutically relevant signs easily and quickly. It will serve as an invaluable guide to the practice of this new form of visual medicine.

Abstract

This work aims to provide a detailed reasoned map of fetal behavior by describing its features and evolution from 10 to 25 weeks gestation. Each behavioral event is described, measured in seconds and presented as it occurs in its natural sequence, thus providing a ‘real-time’ picture of fetal behavior. Links between events become clear, and events are connected to the changing anatomy and physiology of the nervous system and of the fetal body in general. Transient behaviour with its underlying structures and possible functions is carefully distinguished from anticipatory functions heralding post-natal life. Furthermore, phenomena of physiologic significance during prenatal life, but which can acquire pathological connotations for the premature infant having to meet the different requirements of the post-natal environment, are also outlined. Finally, various prenatal ‘myths’ are discussed, in order to give scientific information on issues;that are particularly relevant, given the heated pro-life versus pro-choice debate.

La possibilità di far riferimento ad apparecchiature a 16 o 64 file di detettori con acquisizioni volumetriche ottenute in pochi secondi, associata all’utilizzo del mezzo di contrasto e ad elaborazioni delle immagini con work-station in cui la trasmissione è immediata, costituisce la premessa per risolvere le complesse problematiche diagnostiche riguardanti l’emergenza-urgenza e fornire il relativo referto radiologico in tempi estremamente contenuti. Lo scopo di questo testo è di fornire al Radiologo, attraverso la presentazione di casi clinici, una metodologia rigorosa di applicazione dei protocolli di studio con mezzo di contrasto multifasici e consentire, attraverso diagnosi altamente accurate, l’adeguato trattamento terapeutico contribuendo in molte circostanze a "salvare la vita" all’ammalato.

Abstract

Recent advancements in nano/micro materials and related characterization approaches allow the design of a new type of ultrasound contrast agents (UCAs) with enhanced multifunctional behaviour. This is chance is also supported by the recent achievements in modelling and signal processing. This book provides the state of art of the research activity of two successful European projects, TAMIRUT and SIGHT, addressing an integrated system, encompassing the contrast agent, the hardware equipment and the processing strategies, as a key tool for a combined diagnostic and therapeutic approaches (theranostics) in medical ultrasound.
