Abstract

Après le succès de l'ouvrage - TDM des urgences abdominales - l'auteur s'attache ici à décrire de la manière la plus pratique et didactique possible la prise en charge des tumeurs abdominales au scanner. Dans la première partie, le lecteur trouvera une classification histologique générale des tumeurs, des rappels sur la technique TDM appliquée à l'étude abdomino-pelvienne, un système de planches légendées présentant la radio-anatomie de façon multiplanaire ainsi qu'une cartographie TDM relativement inédite des chaînes ganglionnaires abdomino-pelviennes, et enfin les principaux éléments de sémiologie TDM élémentaire présents en pathologie tumorale abdominale. Dans la seconde partie de l'ouvrage, une suite de fiches pédagogiques traitant des tumeurs abdominales est proposée au lecteur. Les tumeurs sont classées par organe puis selon leur caractère bénin ou malin. Pour chaque tumeur sont renseignés les principales données épidémiologiques, l'aspect en anatomo-pathologie, le contexte clinique, la sémiologie en TDM, l'intérêt des autres techniques d'imagerie, les principaux diagnostics TDM différentiels, l'évolution naturelle, le pronostic et les grandes lignes de prise en charge. Abstract

In Canada, stroke is the third most common cause of death with an estimated incidence of approximately 50,000 cases each year. The majority of strokes are ischemic (80%) and are a result of interruption of blood flow to the brain caused by a blood clot. Transient Ischemic Attack (TIA) is defined as "a transient episode of neurodysfunction caused by focal brain, spinal cord, or retinal ischemia, without acute infarction". Approximately 15,000 Canadians experience a TIA each year and are five times more likely to suffer from a stroke in the following two years when compared to the general population. The need for an accurate and timely diagnostic test is therefore needed, as the risk of recurrent stroke within 90 days after a TIA is between 10% to 20%. The combination of imaging modalities, laboratory tests, and clinical features provide a comprehensive diagnosis for stroke. Advances in imaging technology such as computed tomography (CT) and magnetic resonance imaging (MRI) have allowed for the distinction between hemorrhagic and ischemic strokes, providing clinicians with greater certainty whether to administer thrombolysis. With rapid scan times and ease for detecting intracranial hemorrhage, CT has shown to be a preferred diagnostic technology. MRI has been shown to easily detect ischemia with diffusion-weighted imaging and also detects hyperacute hemorrhage with proper sequences. MRI is not as widely available as CT and is typically more costly. With no single gold standard for the diagnosis of stroke, uncertainty regarding the appropriateness of solely using CT or expert clinical assessment remains. Furthermore, current clinical practice guidelines state that CT tests must be performed, though this technology is not always available in rural or remote emergency departments. The objective of this review is to summarize recent clinical evidence of CT and expert clinical assessment to support the diagnosis of stroke and TIAs.

Abstract

The availability and diffusion of high-performance technologies has strengthened the role of CT angiography and MR angiography as simple, reliable techniques for the characterization and treatment planning of the principal diseases of the cardiovascular system, an understanding of which has become essential for all radiologists. The aim of this book is to provide concise but thorough information and guidance on the main methods and examination techniques used when performing high-quality CT angiography and MR angiography studies in a broad range of clinical settings. Abstract

Respiratory motion causes an important uncertainty in radiotherapy planning of the thorax and upper abdomen. The main objective of radiation therapy is to eradicate or shrink tumor cells without damaging the surrounding tissue by delivering a high radiation dose to the tumor region and a dose as low as possible to healthy organ tissues. Meeting this demand remains a challenge especially in case of lung tumors due to breathing-induced tumor and organ motion where motion amplitudes can measure up to several centimeters. Therefore, modeling of respiratory motion has become increasingly important in radiation therapy. With 4D imaging techniques spatiotemporal image sequences can be acquired to investigate dynamic processes in the patient’s body. Furthermore, image registration enables the estimation of the breathing-induced motion and the description of the temporal change in position and shape of the structures of interest by establishing the correspondence between images acquired at different phases of the breathing cycle. In radiation therapy these motion estimations are used to define accurate treatment margins, e.g. to calculate dose distributions and to develop prediction models for gated or robotic radiotherapy. In this book, the increasing role of image registration and motion estimation algorithms for the interpretation of complex 4D medical image sequences is illustrated. Different 4D CT image acquisition techniques and conceptually different motion estimation algorithms are presented. The clinical relevance is demonstrated by means of example applications which are related to the radiation therapy of thoracic and abdominal tumors. The state of the art and perspectives are shown by an insight into the current field of research. The book is addressed to biomedical engineers, medical physicists, researchers and physicians working in the fields of medical image analysis, radiology and radiation therapy.
