Abstract

This book explores a number of issues related to the stigma arising from HIV/AIDS infection, perceived or actual discrimination from the community and society, and the extent of vulnerabilities for infected Asian refugees and immigrants. It assesses the health care and treatment regimen for HIV/AIDS accessed by immigrants and refugee claimants in North America, including treatments offered by the health-care system and ethnic communities, and their perceptions and biases relating to HIV/AIDS issues. On another level, the book identifies the ways in which HIV-sufferer immigrants and refugees/refugee claimants from Asia are vulnerable to discrimination due to 1) lack of information about HIV/AIDS incidence in the community; 2) inability of the health system to respond appropriately; and 3) the community’s need for introspection on their own health issues. This book reveals the dynamics that influence choice, behavior, and lifestyle of HIV sufferer immigrants, adds to the existing knowledge about refugees and migrants, and proposes a unified theory of discrimination and stigmatization within the context of human rights. This Brief describes the concept and realization of gene therapy for HIV from the unique historic perspective and insight of two pioneers of the clinical applications of stem cell gene therapy for HIV. Gerhard Bauer applied ribozyme-anti-HIV and other vectors to manufacture clinical grade, HIV-resistant hematopoietic stem cells for the first patients that received stem cell gene therapy for HIV, including the first child in the world and the first fully marrow-ablated HIV infected patient. Joseph Anderson developed the most recent and most potent combination anti-HIV lentiviral vectors and pluripotent stem cell applications for HIV gene therapy and tested these in the appropriate in vitro and vivo models, paving the way for novel HIV gene therapy approaches to possibly cure patients. In Gene Therapy for HIV, Bauer and Anderson discuss the unique aspects of this therapy, including its limitations and proper safety precautions and outline a path for a possible functional cure for HIV using stem cell gene therapy based on a cure already achieved with a bone marrow stem cell transplantation performed in Germany using donor stem cells with a naturally arising CCR5 mutation. In addition, the Brief provides a thorough and methodical explanation of the basics of gene therapy, gene therapy vector development, in vitro and in vivo models for HIV gene therapy and clinical applications of HIV gene therapy, including Good Manufacturing Practices.

Abstract

The association between AIDS and cancer was recognized from the beginning ofthe AIDS epidemic, when the appearance of Kaposi sarcoma in a cluster of youngmen was one of the first signs of this new disease. It was soon recognized that AIDSpatients are prone to develop a number of “AIDS-defining” cancers: Kaposi sarcoma,lymphoma, and cervical cancer. The development of effective combination anti-HIVtherapy starting around 1996 converted AIDS from a death sentence to a manageabledisease and led to dramatic shifts in the epidemic. As this therapy was able to improveimmune function in patients, the incidence of most “AIDS-defining” cancers decreased.There is a misconception, however, that AIDS has gone away. In fact, as AIDS patientsare living longer, the number of AIDS patients has more than doubled in the UnitedStates since 1996, and the AIDS population overall has increased in age. Also, as AIDSpatients are less likely to die of other complications, cancer is coming to the forefront as one of the most common causes of death in regions where AIDS drugs are widelyavailable.Moreover, the three “AIDS-defining” cancers are now taking a back seat toa number of other HIV-associated cancers, such as Hodgkin lymphoma, lung cancer,and anal cancer. In the developing world, AIDS-associated cancers are a major publichealth problem, and in some regions of sub-Saharan Africa, Kaposi sarcoma is themost common tumor in men. In recent years, there has been a vast increase in our understanding of HIV-associatedcancers. We now know, for example, that most are caused by other viruses and thatthe main role of HIV and immunodeficiency is to provide a supportive environmentfor the viruses to multiply and for the cancers to develop. But there remain a numberof unanswered questions and a need for improved prevention and therapy. In the 28chapters of this book, written by some of the most renowned experts in this field, wepresent up-to-date information on the cancers associated with HIV infection. Thechapters cover the epidemiology of these cancers, as well as their pathogenesis, clinicalpresentation, and treatment.
