5-fluorouracil (5-FU) is one of the most widely used cytotoxic agents. especially in gastrointestinal cancers. Protracted venous infusion is a com mon-place route of drug administration but imply some limits. Patients seem to prefer treatment by oral intake but not at the cost of antitumoral activity.The absence of intravenous access devices and complications associated with their implantation is a major benefit of oral fluoropyrimidines. S-1 is a combination of three pharmacological compounds : tegafur, gimeracil and oteracil. Tegafur is a prodrug of 5-FU, an oral fluoropyrimidine, and has been developed as a replacement for infusional 5-FU chemotherapy. S-1 based therapy is a recommended first-line treatment for advanced gastric cancer in Japan since 1999. Following a clinical trial in 24 non-asian countries, S-1 in combination with cisplatin has been approved in Europe in 2011 and has shown equivalent efficacy and a better tolerance comparing to 5-FU.Furthermore, S-1 has a favorable toxicity (besides hand-foot syndrome). In spite of this convenient use, S-1 needs probably more attention to patient education and patient support to optimize compliance and safety. Le 5-fluorouracile (5-FU) est un des agents cytotoxiques les plus employés, en particulier dans les cancers digestifs. La perfusion continue est un mode d'administration banale mais comporte certaines limites. Les patients semblent préférer un traitement par voie orale, mais pas au détriment d'une activité anti-tumorale . L'absence d'un dispositif intraveineux et de complications associées à son implantation constitue un atout majeur des fluoropyrimidines orales. S-1 est une association de trois composés pharmacologiques :tégafur, giméracil et otéracil. Tégafur est un prodrug du 5-fluorouracile (5-FU), une fluoropyrimidine orale, et a été développé pour remplacer une chimiothérapie à base de 5-FU en perfusion continue. La chimiothérapie à base de S-1 est considérée comme traitement de première ligne pour le cancer gastrique avancé inopérable au Japon depuis 1999. Suite à une étude clinique dans 24 pays non asiatiques, S-1 associé au cisplatine a été approuvé en Europe en 201 1 et a montré une efficacité équivalente et une meilleure tolérance par rapport au 5-FU. En outre, le profil de toxicité s'est avéré favorable (hormis le syndrome mains-pieds). Malgré son confort d'utilisation, S-1 nécessite probablement plus d'attention à l'éducation et au soutien des patients en vue de garantir une meilleure compliance et une meilleure sécurité des patients.
