Abstract

This open access book about the Zadeh Project demonstrates and explores a core question in clinical ethics: how can ethics consultants be accountable in the face of a robust plurality of ethical standpoints, especially those that underwrite practices and methods for doing ethics consultation as well as those viewpoints and values encountered in daily clinical ethics practice? Underscoring this question is the recognition that the field of clinical ethics consultation has arrived at a crucial point in its maturation. Many efforts are underway to more formally “professionalize” the field, with most aimed toward stabilizing a specific set of institutional considerations. Stretched between these institutional and practical initiatives resides a crucial set of of ethical considerations, chief among them the meaning and scope of responsibility for clinical ethics consultants. Developed around a long-form case scenario, the Zadeh Project provides a multi-layered series of “peer-reviews”: critique of the actions of the case scenario’s ethics consultant; reflection on clinical ethics method; examination of the many ways that commitments to method and practice can, and do, intersect, overlap, and alter one another. Abstract

Hyperbaric oxygen treatment (HBO2) is a widely accepted adjuvant therapy in various health conditions that exhibit impaired tissue blood flow. At high pressures, the delivery of the dissolved oxygen in plasma is enhanced, which contributes to better tissue oxygenation, cellular metabolism and ultimately, healing. However, this is not the only beneficial outcome of HBO2 treatment since oxygen is a highly reactive molecule and can induce upregulation of many enzymatic systems in the cell at the cellular, genetic and molecular level. Particularly, vascular/endothelial function is affected by the HBO2. Our understanding of these mechanisms is still emerging. There have been many controversies related to the HBO2 protocols and indications. As well as exhibiting beneficiary effects on the tissue perfusion, it is known that HBO2 demonstrates high toxicity at higher pressures, due to increased oxidative stress and barotrauma. On the other hand, there is a lack of translation of the knowledge on the mechanisms of action of HBO2 obtained from the experimental research to the clinical practice. Thus, this book presents the reader with an overview of the current knowledge on the mechanisms of HBO2 effects in various experimental models and clinical treatment protocols, in an attempt to provide a better understanding of how and when HBO2 should be used as an effective therapy without unwanted side effects.

Abstract

This book critically analyses the influence of international policies and guidelines on the performance of interventions aimed at reducing health inequities in Latin America, with special emphasis on health promotion and health in all policies strategies. While the implementation of these interventions plays a key role in strengthening these countries’ capacity to respond to current and future challenges, the urgency and pressures of cooperation and funding agencies to show results consistent with their own agendas not only hampers this goal, but also makes the territory invisible, hiding the real problems faced by most Latin American countries, diminishing the richness of local knowledge production, and hindering the development of relevant proposals that consider the territory’s conditions and cultural identity. Departing from this general analysis, the authors search for answers to the following questions: Why, despite the importance of the theoretical advances r egarding actions to address social and health inequities, haven’t Latin American countries been able to produce the expected results? Why do successful initiatives only take place within the framework of pilot projects? Why does the ideology of health promotion and health in all policies mainly permeate structures of the health sector, but not other sectors? Why are intersectoral actions conjunctural initiatives, which often fail to evolve into permanent practices?
