Abstract

This text provides a comprehensive review and expertise on various interventional cancer pain procedures. The first part of the text addresses the lack of consistency seen in the literature regarding interventional treatment options for specific cancer pain syndromes. Abstract

La gestion de la douleur post-opératoire des veaux ébourgeonnés au fer chaud a été largement étudiée dans la littérature. S’il y a un consensus sur l’intérêt de l’anesthésie locale au moment de l’ébourgeonnage (EB), les auteurs ne sont pas unanimes quant à celui de l’administration d’un anti-inflammatoire non-stéroïdien. Les études différant toutes par leur format, elles sont difficilement comparables. Pour notre expérience, 24 veaux Holstein ont été séparés en 3 groupes de 8 veaux, recevant tous un anesthésique local avant l’EB. Un groupe a reçu du meloxicam au moment de l’EB. Un autre groupe en a reçu au moment de l’EB, puis à nouveau 3 jours plus tard. Le dernier groupe n’en a pas reçu. Les veaux ont été observés 2 fois par jour. Des paramètres comportementaux, cliniques et zootechniques ont été mesurés. L’originalité de cette expérience tient au fait qu’elle a été effectuée en conditions de terrain, avec un suivi de 6 jours post-opératoires. Au terme de l’expérience, les analyses statistiques n’ont montré aucun effet du meloxicam sur ces paramètres, malgré des tendances graphiques pour certains. Cela ne permet pas de conclure à un réel bénéfice du meloxicam dans la gestion de la douleur chez le veau après EB au fer chaud, que ce soit après une ou deux injections.

Abstract

This book discusses the role of placebos and nocebos in the treatment of headache disorders. These disorders are usually treatable, but safety and tolerability issues mean that available preventive treatments have often limited success, even in the right hands – one in five patients treated with a migraine preventive pharmaceutical agent discontinues treatment for those reasons. The nocebo effect plays a role here, with patients’ negative expectation and previous unpleasant treatment experiences creating negative belief in the treatment’s benefits and safety, which in turn limits treatment outcomes and adherence significantly. In RCTs on migraine prevention, one in 20 patients treated with a placebo discontinued treatment because of adverse events, indicating a considerable nocebo effect; the fewer potential adverse events described in the consent form, the smaller the nocebo effect. As such, physicians treating headache sufferers should acknowledge nocebo as a significant cofactor for treatment adherence and failure, and plan techniques to limit the effects, such as patient education and close follow-up. This highly informative and painstakingly presented book provides scientific insights for professionals and scholars with an interest in internal medicine, neurology and pain medicine. .
