Abstract

The Philippines has made remarkable progress in the last decades in improving its health outcomes. However, performance is still poor compared regionally and with similar economies. This report was developed with the aim of informing health policy during the political transition from the Autonomous Region in Muslim Mindanao (ARMM) to the Bangsamoro Autonomous Region in Muslim Mindanao (BARMM). This transition began with the ratification of the Bangsamoro Organic Law (BOL) in January 2019. This report provides an overview of the current capacities and challenges facing the public primary health care system as the BARMM government enters into its new role. By providing a detailed look at the availability of inputs needed to deliver priority services, it highlights current strengths, and also indicates areas that need to be strengthened if the national and subnational health goals are to be realized. Historically, health outcomes have been far worse in BARMM than elsewhere in the country. While health outcomes are complex and multi-dimensional, weaknesses in health governance have exacerbated the geographic and socio-cultural challenges associated with health care delivery. While the authors recognize that the transition is likely to introduce some challenges, it is hoped that the findings documented in this report can guide health policy, including investments towards high-impact priorities and in this way, help lay the foundation for better health care, and better health for the people of BARMM.

Depuis les années 2000, la santé de la reproduction semble constituer un sujet d'inquiétude en Inde. Les taux de mortalité maternelle et infantile encore élevés discréditent l'image de superpuissance que l'État aime afficher, le déséquilibre du sex-ratio continue de se creuser et, malgré une importante baisse du taux de fécondité, le pays doit faire face à une population de plus d'un milliard trois cent millions d'habitants. À partir d'une enquête de terrain d'un an et demi dans un hôpital public et dans les bidonvilles de Jaipur, Clémence Jullien analyse les conséquences, pour les femmes et leur famille, des nouveaux programmes de santé : une prime financière incite les femmes à accoucher à l'hôpital plutôt qu'avec des accoucheuses traditionnelles et, depuis 2011, les soins obstétriques à l'hôpital sont devenus entièrement gratuits. Toutefois, ces programmes, censés garantir l'accès aux soins, rendent les bénéficiaires les plus vulnérables davantage conscients des inégalités socio-économiques qu'ils subissent, renforcent les stéréotypes existants et donnent au personnel hospitalier et aux membres d'ONG un pouvoir discrétionnaire. Tensions sociales (castes, classes) et religieuses se cristallisent autour de la maternité. D'autres enjeux cruciaux - discrimination à l'égard des petites hiles, faible pouvoir décisionnel des femmes, recours limité à la contraception - surgissent alors, accentuant les différences au sein de la société indienne, sous couvert de progrès et au nom de l'intérêt de la nation.

Han transcurrido más de dos decenios desde la última vez que la Organización Mundial de la Salud (OMS) publicó un documento de orientación técnica sobre la atención de las embarazadas sanas y sus bebés, Cuidados en el parto normal: una guía práctica, y el panorama mundial de los servicios de maternidad ha cambiado considerablemente desde entonces. Actualmente hay más mujeres que dan a luz en centros de atención de salud en muchas partes del mundo, sin embargo, la calidad insuficiente de la atención prestada sigue impidiendo el logro de los resultados deseados en materia de salud. Mientras que en algunos entornos se realizan muy pocas intervenciones que llegan demasiado tarde, en otros las mujeres reciben demasiadas intervenciones que se realizan demasiado pronto.

Simulation is becoming an integral part of medical education and has already a well-established role within anaesthesia training, with many examination questions already modelled on simulation scenarios.
