The definitive guide to thoracic spine pathologies and state-of-the-art surgical approachesSurgery of the Thoracic Spine: Principles and Techniques by renowned spine surgeons Ali Baaj, Kumar Kakarla, and Han Jo Kim fills a gap in the literature, with content focused solely on pathologies and surgical techniques of the thoracic spine and vertebral column. Starting with a thoughtful discussion on the uniqueness of the thoracic region as it relates to pulmonary function, the richly illustrated textbook covers a full spectrum of topics from biomechanics and anesthetic considerations to neuromonitoring and neuronavigation. With contributions from a cadre of distinguished experts, the book encompasses pathophysiology, surgical techniques, and reconstructive strategies for common degenerative, congenital, oncologic, and traumatic diseases of the thoracic spine. Dedicated chapters cover treatment options for different types of scoliosis, Scheuermann kyphosis, proximal junctional deformity, and posttraumatic deformity.Key FeaturesTreatment of common degenerative conditions including stenosis and disc herniationsManagement of less common inflammatory and infectious spinal diseases such as spondylarthropathies, osteomyelitis, discitis, and fungal and tubercular infectionsOncologic topics including primary, intradural extramedullary, and intramedullary spinal cord tumors and thoracic spine metastasesSurgical treatment of pediatric and adult deformities including congenital, idiopathic, and degenerative scoliosisClassification of thoracic spinal fractures, discussion of complete and incomplete thoracic spinal cord injuries, posterior and ventral treatment of thoracic spine fractures, and osteoporotic compression fracturesThis is an invaluable evaluation and management tool for neurosurgical and orthopaedic residents and practicing spine surgeons who treat patients with common to complex thoracic spinal pathologies.

This comprehensive, up-to-date textbook of modern cervical spine surgery describes the standard and advanced techniques recommended by the Cervical Spine Research Society – European Section (CSRS-E) with a view to enabling both young and experienced surgeons to further develop their skills and improve their surgical outcomes. Success in cervical spine surgery depends on the surgeon’s awareness of the main challenges posed by distinct cervical spine diseases, theoretical understanding of treatment concepts, and knowledge of technical options and the related potential for complications. It is the surgeon who has to merge theory and practice to achieve the desired outcome, in each case appraising the details of surgical anatomy and weighing the challenges and complications associated with a surgical technique against the skills that he or she possesses. This excellently illustrated book, written by key opinion makers from the CSRS-E with affiliated surgeons as co-authors, presents the full range of approaches and techniques and clearly identifies indications, precautions, and pitfalls. It will be a superb technical reference for all cervical spine surgeons, whether orthopaedic surgeons or neurosurgeons.

Surgical Anatomy of the Lumbar Plexus is the only book on the market devoted to the lumbar plexus and its branches, focusing on anatomy and clinical applications, pathology, surgery, and imaging. Internationally known authors R. Shane Tubbs, Marios Loukas, Amgad Hanna, Rod Oskouian and a cadre of esteemed specialists provide unique insights, clinical pearls, knowledge based on thousands of spine surgeries, and a well-rounded multidisciplinary perspective. Nine separate chapters dedicated to impacted nerves: subcostal, iliohypogastric, ilioinguinal, lateral femoral cutaneous, genitofemoral, femoral, obturator, furcal, and accessory obturator. History and overview of dermatomal anatomy of branches derived from the lumbar plexus. Anesthetic blockade of the femoral, lateral femoral cutaneous, iliohypogastric, ilioinguinal, and genitofemoral nerves. Tips for preventing complications and achieving positive outcomes for lateral transpsoas approaches to the lumbar spine and other procedures. -- Publisher

This book is intended to serve as a comprehensive, up-to-date resource for clinicians involved in the management of patients with cervical degenerative disease. The text is divided into sections, organized in a clinically strategic manner. Initial chapters address the basics of cervical spine anatomy and biomechanics as well as the pathophysiology leading to various cervical degenerative disorders and the possible neurological sequelae. Subsequent chapters outline characteristics of the clinical presentation and the various diagnostic modalities to evaluate these patients. Key elements involved in the surgical-decision making process are covered, providing the necessary elements to establish a solid foundation for treatment planning. The final sections discuss specific procedures; including traditional approaches as well as more recent developments such as motion preservation surgery and minimally invasive techniques. Individual chapters are organized with an introductory outline containing key chapter elements. Chapters focusing on specific pathological entities include discussions regarding pathophysiology, genetics, and risk factors. Those describing surgical procedures include a discussion on indications/contraindications, pre-operative planning, surgical technique, post-operative care, and complication avoidance. Chapter authors present their personal experience enhancing the information from current, evidence-based, referenced material. When appropriate, case presentations are added to provide a practical application of chapter’s key points.

This issue of Acta Neurochirururgica presents the latest surgical and experimental approaches to the craniovertebral junction (CVJ). It discusses anterior midline (transoral transnasal), posterior (CVJ craniectomy laminectomy, laminotomy, instrumentation and fusion), posterolateral (far lateral) and anterolateral (extreme lateral) approaches using state-of-the-art supporting tools. It especially highlights open surgery, microsurgical techniques, neuronavigation, the O-arm system, intraoperative MR, neuromonitoring and endoscopy. Endoscopy represents a useful complement to the standard microsurgical approach to the anterior CVJ: it can be used transnasally, transorally and transcervically; and it provides information for better decompression without the need for soft palate splitting, hard palate resection, or extended maxillotomy. While neuronavigation allows improved orientation in the surgical field, intraoperative fluoroscopy helps to recognize residual compression. Under normal anatomic conditions, there are virtually no surgical limitations to endoscopically assisted CVJ and this issue provides valuable information for the new generation of surgeons involved in this complex and challenging field of neurosurgery.
