Devenue un fait de société par le nombre de personnes qu'elle touche et par les débats que ses développements suscitent, la Procréation Médicalement Assistée repose d'abord et avant tout sur des réalités biomédicales. Loin du forum bioéthique, la fivete, ses corollaires et dérivés, ont comme unique objectif de permettre à des couples stériles de connaître les joies de la conception alors que les réalités anatomiques, endocriniennes ou autres le leur interdisent spontanément. Rédigée par un biologiste, la fertitité contrariée ne se contente pas de faire un inventaire des recettes ni des traitements médicaux réparateurs. Elle rappelle d'abord les réalités génétiques, biochimiques et physiologiques qui autorisent la conception. Elle aborde ensuite avec méthode les alternatives thérapeutiques qui permettent d'espérer une conception lorsqu'un ou plusieurs paramètrres, chez l'homme, la femme ou les deux, pêchent par défaut ou par excès. De la mise au point clinique à la fivete, l'auteur aborde les différentes ressources thérapeutiques offertes aujourd'hui par les PMA en en détaillant le " pourquoi " et le " comment ", les avantages mais aussi les limites. Sans concession et avec le recul pondéré dû à plus de dix ans de pratique quotidienne. Le texte est assorti d'un lexique qui offre une définition à près de 300 mots et acronymes liés aux PMA et à la reproduction en général. L'ouvrage apparaît donc comme un recours nécessaire sinon indispensable pour toute personne concernée par cette problématique, que ce soit à titre personnel ou professionnel.

The present work tries to show that problematic evaluations of new technologies such as IVF may be as dangerous as no evaluation at all. Because technology assessments may give people a false sense of security, poor ones may serve as a legitimization of premature, dangerous, or misunderstood decisions. Careful analysis of technology assessments may uncover particular epistemological and ethical problems that could misguide public policy. This book is the first to offer an extensive evaluation of four prominent institutional assessments of IVF. This philosophical analysis revels how inadequate assessments may produce policies that are not in the public's best interests. (Book jacket)

Preimplantation genetic diagnosis (PGD) is taken to mark the starting-point of a new phase in human reproduction, where the possibility of choosing children on genetic grounds without having to resort to dangerous or ethically controversial procedures (such as abortion) will gradually increase. Ethical and political issues actualised by this development are addressed. The discussion touches upon issues regarding the moral status of embryos and gametes, the moral import of respecting individual autonomy and its implications for the requirement of informed consent in health-care, the connection between sickness, disability and the value of life, the moral status of possible future people, and the connection between choosing children and eugenic policies of the past. Practical policy issues are adressed on the basis of this as well as an empirical case-study of the introduction of PGD in Sweden. The book ends up in a set of recommendations regarding the management of research on, introduction and routine use of pure selection, both within health care and from the point of view of society as a whole. It is argued that research on such procedures should be allowed and supported by society. However, tight restrictions regarding the clinical introduction of new procedures in this area is highly desirable. A rough model for implementing such restrictions is also presented. It is further asserted that, although reasons of economy and safety should limit the access to pure selection, society should not apply any explicit restrictions based on ideas regarding how different traits affect a person's quality of life. It is stressed that, in order to avoid a resurrection of eugenic policies of the past, the development in this field underlines the need for continued and strengthened public support to the sick, disabled and mentally retarded.

Questions éthiques, psychologiques, juridiques et scientifiques. Les personnes, les couples, les familles s'interrogent sur les nouvelles formes de parentalité et souhaitent développer et mettre à jour leur information sur les réponses données à l'infertilité. La sensibilisation aux aspects humains de ce problème, plus particulièrement auprès des professionnels de la procréatique et de l'adoption, conditionne l'évolution des approches existantes vers une prise en compte plus complète de la question. Le douloureux problème de l'infertilité est abordé parfois pour le résoudre, parfois pour lui ouvrir de nouvelles perspectives, par des auteurs particulièrement compétents et avisés. Ils traitent le sujet de l'infertilité et de ses recours: la procréation médicalement assistée, l'adoption... Ils envisagent, chacun selon sa formation, les aspects scientifiques, cliniques, éthiques, psychologiques et juridiques. Ces textes nourrissent le débat à ces différents niveaux et éclairent la réflexion au sein de l'opinion. La pluralité des opinions exprimées contribue au progrès de la concertation des différentes tendances existantes et suscitera une saine collaboration entre les instances concernées.
