Abstract

Trata do papel dos movimentos sociais em saúde no processo de reformulação da política de saúde, plasmado na Constituição de 1988 e denominado de Reforma Sanitária brasileira. Os percalços da sua implementação são analisados no contexto do processo de democratização do País ocorrido nos últimos 10 anos. A autora contribui para a interpretação da problemática da saúde no Brasil contemporâneo, sem a qual não se pode falar em democracia e cidadania. Através da leitura deste trabalho, os cientistas políticos e os profissionais de saúde poderão refletir acerca do lugar que ocupam no sistema e nos serviços de saúde, bem como das articulações de suas representações com a política de saúde.

Abstract

Involving citizens in policy decision-making processes - deliberative democracy - has been a central goal of the Labour government since it came to power in 1997. But what happens when members of the public are drawn into unfamiliar debate, with unfamiliar others, in the unfamiliar world of policy making at national level? This book sets out to understand the contribution that citizens can realistically be expected to make. Drawing on the lessons from an ethnographic study of a public involvement initiative in the health service - the Citizens Council of NICE (National Institute for Clinical Excellence) - the book explores the practical realities behind the much-quoted faith in 'deliberation' that underpins so many models of public involvement and presents the analysis of sixty four hours of video and audiotape capturing a warts-and-all picture of deliberation in action. It sets deliberative participatory initiatives within a broad inter-disciplinary context and challenges politicians, policy-makers and academics to develop more realistic approaches to democratic innovation. Citizens at the centre will be of interest to academics and students in social policy, sociology, politics, health, social care, economics, and public administration and management. Abstract

Why do some developing countries have more efficient health systems and better health outcomes? Contrary to existing theory that posits the superiority of proportional representation (PR) rules on public-goods provision, this book argues that electoral rules function differently given the underlying ethnic structure. In countries with low ethnic salience, PR has the same positive effect as in past theories. In countries with high ethnic salience, the geographic distribution of ethnic groups further matters: where they are intermixed, PR rules are worse for health outcomes; where they are isolated, neither rule is superior. The theory is supported through a combination of careful analysis of electoral reform in individual country cases with numerous well-designed cross-country comparisons. The case studies include Thailand, Mauritius, Malaysia, Botswana, Burma and Indonesia. The theory has broad implications for electoral rule design and suggests a middle ground in the debate between the Consociational and Centripetal schools of thought.

Abstract

Within the last twenty years a large-scale bottom-up privatization has taken place in Vietnam, changing and dismantling the public health care system. This process has led to severe tensions inherent in the transitional society of Vietnam between equity and access to health care support - especially for the poor, elderly, migrants, and ethnic minorities - on the one hand, and its efficiency on the other hand. The book traces the reform efforts to modernize the health care system by the Communist Party of Vietnam and the Vietnamese government. The author bases her findings on little known primary literature and interviews with key stakeholders of the policy network involved in the reform of the health care system, thereby painting an authentic atmospheric picture of the profound changes in the health care system in Vietnam.
