Abstract

Poor Families in America's Health Care Crisis examines the implications of the fragmented and two-tiered health insurance system in the United States for the health care access of low-income families. For a large fraction of Americans their jobs do not provide health insurance or other benefits and although government programs are available for children, adults without private health care coverage have few options. Detailed ethnographic and survey data from selected low-income neighborhoods in Boston, Chicago, and San Antonio document the lapses in medical coverage that poor families experience and reveal the extent of untreated medical conditions, delayed treatment, medical indebtedness, and irregular health care that women and children suffer as a result. Extensive poverty, the increasing proportion of minority households, and the growing dependence on insecure service sector work all influence access to health care for families at the economic margin.

Abstract

Shortly after the dawn of the twentieth century, the New York City Department of Health decided to address what it perceived as the racial nature of health. It delivered heavily racialized care in different neighborhoods throughout the city: syphillis treatment among African Americans, tuberculosis for Italian Americans, and so on. It was a challenging and ambitious program, dangerous for the providers, and troublingly reductive for the patients. Nevertheless, poor and working-class African American, British West Indian, and Southern Italian women all received some of the nation’s best health care during this period.Health in the City challenges traditional ideas of early twentieth-century urban black health care by showing a program that was simultaneously racialized and cutting-edge. It reveals that even the most well-meaning public health programs may inadvertently reinforce perceptions of inferiority that they were created to fix.

Abstract

Food insecurity is a complex ‘wicked’ problem that results from a range of unstable and uncertain physical, social, cultural and economic factors that limits access to nutritious food. Globally, 800 million people are under-nourished, and around 2 billion are overweight/obese or have micronutrient deficiency. These populations are largely positioned in developing countries where disease burden is high and impacts health budgets and productivity. Similarly developed countries, cities and neighbourhoods are experiencing a greater emergence of vulnerable populations. This is in part explained by the change in the food production and manufacturing, the retraction in economic climates, the increase in food price, and in some regions reduced food availability and access.Vulnerable groups include but are not limited to migrant populations, Indigenous people, elderly, pregnant women, those with disability, homeless, young children and youth. Poor nutrition at significant periods of growth and development and during life impact long term health outcomes increasing non-communicable disease prevalence, health cost and reducing economic productivity.
