Abstract

In order to cope with the growing demand for hospitalisation and to improve patient satisfaction, alternatives to traditional hospitalisation are being developed all over the world. One of them is the hospital at home.&#13;Therefore, the objective of this thesis is to highlight promising research questions in the area of tactical planning for the hospital at home and to see what implications this has at the short-term level. More specifically, this thesis aims to answer the question: How can tactical planning models for the hospital be adapted to take into account the case of the hospital at home and what are the impacts at the operational level?.&#13;To this end, the thesis will begin with a literature review that will focus on decisions made in hospitals and home health care institutions. All three levels of decisions will be studied, but particular attention will be paid to the tactical level. Then, more information about hospital at home will be given. This part will include a reminder of the differences between home health care and hospital at home. The development of hospital at home in several countries will also be analysed. Finally, the results of the interviews conducted in the framework of this thesis will be presented. These interviews were carried out in various Belgian hospitals offering a service of hospital at home. The last part of the thesis will discuss the adaptations to be made to the hospital’s decision‐making models when the case of hospital at home is considered and will also address the impact of these modifications on the operational level.&#13;As a result, it was seen that there were fewer changes at the tactical level than expected. But that the few changes impacted more strongly on the operational level. Moreover, the changes mainly concern nurses, who are one of the most important resources of the hospital. In addition, the hospital at home could increase the capacity to admit patients, but on the other hand, the hospital would be faced with a lack of staff. At the tactical level, it is therefore wise to think carefully about the selection of shifts and the number of staff per shift, so as not to complicate the daily/weekly management of the nurses.

Abstract

Ces dernières années, les drones ont suscité un intérêt massif dans le contexte de la livraison du dernier kilomètre. Ces livraisons représentent la partie la plus coûteuse de la logistique d'un produit et sont responsables d'importantes émissions de CO2. L'introduction des drones dans la logistique permettrait non seulement de livrer des colis en moins de 30 minutes, mais aussi de réduire les coûts et les émissions de CO2 qui en découlent. Encore au stade de prototypes, il est difficile d'affirmer ces hypothèses et c'est pourquoi cette thèse étudie les coûts et les impacts environnementaux des drones de livraison.

Abstract

Malaria is a major public health concern in Africa. The lack of human resources and capacity at national levels is one of the main obstacles to the development, implementation and sustainability of malaria control. The management of logistics systems remains a significant weakness. Cameroon is one of the countries on the continent most affected by the disease. However, literature is scarce in the Cameroonian context, concerning the management of the product used for preventing and treating the disease throughout the distribution channel.&#13;This study aims to explore the measures that can be implemented to enhance the supply chain management for antimalarial commodities within Cameroon. The objectives encompass assessing supply chain challenges within the African public health setting, analysing identified issues in the malaria treatment and prevention supply chain in Africa, and proposing actionable measures to improve the supply chain management of antimalarial products in the surveyed country.&#13;A comprehensive investigation was carried out, gathering information from several stakeholders involved in the Cameroonian healthcare supply chain sector. The methodology consisted of a combination of semi- and unstructured interviews to identify the current practices, challenges and potential solutions. The interviews covered eight topics: procurement and sourcing, inventory management, distribution networks, supply chain technology, cold chain management, quality assurance and government regulations, stakeholders’ collaborations and emergency preparedness and response.&#13;The results of the survey demonstrated the importance of reliable data in procurement, real-time inventory tracking, improvements in distribution logistics, the integration of technology to ensure transparency, enhanced cold chain infrastructure and robust quality control systems. Furthermore, the findings revealed the lack of trust in local sourcing, hazardous inventory handling, disparities in distribution across regions, constraints of some communication platforms, and difficulties posed by extreme temperatures.&#13;Strengthening the supply chain management of antimalarial products requires consideration of cooperative initiatives between the public and private sectors. Fostering trust in local goods, promoting fair competition, and adjusting emergency protocols are highly recommended. Yet these goals should not be achieved without considering the equitable distribution of funds, which is an essential element of effective governance.

Abstract

The “Centrale de Services à Domicile” of Huy, Liège, Waremme (CSD) is a non-profit organization which aims at helping people maintain their independence and a high quality of life. One service offered by this structure is to carry people to their non-urgent medical appointments. To achieve this mission, the CSD relies on volunteers, ambulance services or on its own fleet of vehicles which offer collective transport.&#13;As a result of demand growth and difficulties to cope with volunteers, this non-profit organization would like to increase the usage rate of its own fleet of vehicles. However, creating efficient rounds manually is very complex because lots of information has to be taken into account. In the literature, creating rounds to transport people is called a “Dial-a-ride Problem”.&#13;The aim of this project is to propose a computerized solution which will be able to find efficient rounds in a desirable amount of time. This computer program will also help managers taking decisions regarding future actions to undertake in order to make this service grown.&#13;To achieve this goal, we have to get first a deep understanding of how this service is delivered. This part will allow us to set proper parameters in our computer program. Then, we have to carry a literature review in order to find how to deal with dial-a-ride problems. Once a problem-solving method will be chosen, we will implement it in a computer program.&#13;We will demonstrate that the created computer program gives good results and we will be able to give some recommendations on actions to undertake in order to increase usage rate of collective transport. Do we have to open a new supply depot? What kind of vehicles do we have to use? How do we have to organize drivers’ schedules?&#13;However, due to the high complexity of this kind of problem and the number of people to carry via this collective transport, keeping a reasonable computation time was very difficult. So, up to now, only a good initial solution can be found. This computer program needs still some improvements in order to maybe find a better solution. Moreover, it is difficult to validate our conclusions by analyzing several working days. Indeed, gathering information at the CSD is currently not easy.

Abstract

This present thesis is written in order to obtain the Master Degree in Supply Chain Management and Business Analytics of HEC Management School of the University of Liège. It consists of a yearly project involving a detailed analysis of a management issue identified in a company, statements of recommendations and possibly an implementation of these last ones. In our case, the institution studied is the public hospital of Huy, namely Centre Hospitalier Régional de Huy.&#13;Generally speaking, an hospital needs various material flows to run all its caring and other activities. These flows should be efficiently managed and any good must always be delivered at the right place, at the right time and at the right cost in order to provide outstanding quality health care to each patient. The hospital of Huy would like to improve one process of its flow logistics which is currently problematic: the reception of incoming material flows. &#13;This management issue is analysed by developing four chapters. Firstly, we review the particularities of the health domain, the public sector and especially the hospital, its origins, its characteristics and its logistic aspect. Secondly, we describe all the different material flows needed and the traditional process of goods reception in order to understand and explain precisely the situation and the current management issue of the hospital of Huy. Thirdly, three other hospitals of the Walloon Region are benchmarked in order to know their current practices with regards to this step of the supply chain. Finally, after this broad analysis, we formulate one main recommendation to try to solve the problem: setting up a workplace to centralize reception of incoming material flows. All the implying aspects such as the new activities to manage, the processes to respect, the human resources to employ and to train, the infrastructures to adapt and the technological resources to develop are deeply explained as well.&#13;This thesis is a contribution for logistics improvement of the hospital of Huy: it provides its managers with an in-depth transversal organizational analysis and practical recommendations that should be implemented in the future to improve the management of incoming material flows.
