Care for the uninsured is a county responsibility in Florida. This report examines the approaches used by three south Florida counties - Broward, Miami-Dade and Palm Beach - to fund care for the indigent and compares their financing mechanisms, types of services provided, patient travel patterns and costs and productivity across hospital ownership and insurance type.

The authors conducted a workforce study of eye care providers (ophthalmologists, non-ophthalmic physicians, and optometrists) in the United States. Using data from a RAND survey of U.S. ophthalmologists and from publicly available datasets, the authors determined the supply of available providers, and estimated the present and future public health need for eye providers and the current level of demand. The authors then reconciled the supply of providers with present demand and need for services with future need through the year 2010. The study found an overall large surplus of eye care providers in the United States. Which type of provider will have the greatest surplus depends on whether the health care delivery system allocates patients to optometrists or to general ophthalmologists for primary care.

Principles of good governance are discussed, and approaches of other locales to providing health care to the uninsured are summarized. The agency that operates Miami-Dade's health care facilities is also responsible for countywide health care planning, a dual mission that inherently creates a conflict of interest.

An evaluation of Community Voices Miami (CVM), a project aimed at enhancing health care access and quality for the underserved in Miami-Dade County, Florida. The report concludes that CVM affected intermediate outcomes--raising awareness of issues, getting safety-net providers to collaborate, nurturing neighborhood-based solutions, and advocating for establishment of an independent health care planning body; but measurement of ultimate outcomes--access to health care--remains for future study.
