Abstract

This book is a wide-ranging and up-to-date guide to the use of ultrasound for imaging of the thyroid that will meet the needs of novices while providing more experienced professionals with advanced information. All of the techniques employed in modern thyroid ultrasound are covered, from conventional grayscale and Doppler imaging to elastography and contrast-enhanced ultrasound, with a focus on practical aspects and with detailed analysis of their diagnostic value and limitations. The characteristic ultrasound findings of the normal thyroid and a wide range of thyroid diseases and disorders are described with the aid of hundreds of high-quality images. Diffuse changes and thyroid lesions are discussed in depth, paying close attention to aspects of importance for early detection and differential diagnosis. In this context the authors summarize their personal experiences in the use of qualitative parameters of compression elastography and quantitative data of shear-wave elastography. In addition, a practical approach to the risk stratification of thyroid nodules is proposed. The book concludes with a resumé of the main challenges and pitfalls in thyroid ultrasound.

Abstract

This book provides a comprehensive analysis of the value of contrast-enhanced ultrasound (CEUS) in the diagnosis of a wide variety of pathologies. Sonography reliably identifies a wide range of diseases, and the efficacy of modern ultrasound has dramatically improved with contrast enhancement. This book covers almost all aspects of CEUS starting from basic principles and ending with features of its application in individual organs. In particular, it explores the diseases of abdominal, retroperitoneal, and pelvic organs as well as superficial structures, highlighting the characteristic features of typical findings. Focal lesions are discussed in depth, with attention to their early detection and differential diagnosis. Besides, a practical approach to the stratification of the risk of malignancies is provided. The authors summarized their own experience with CEUS in oncology, hepatology, gynecology, urology, endocrinology, and other fields of medicine. This book is based on the authors’ extensive practical experience in the use of modern ultrasound, and other radiological methods, in the diagnosis of thyroid diseases. The authors have analyzed more than 100,000 ultrasound examinations performed between 1995 and 2008 in patients with thyroid and parathyroid disease, as well as many thousands of diagnostic and therapeutic ultrasound-guided minimally invasive procedures. The opening chapters include discussion of current ultrasound techniques, pitfalls, and the specifics of ultrasound examination of the thyroid in children. Detailed attention is then devoted to findings in the normal thyroid and in the presence of diffuse and focal changes. Further chapters focus on such topics as ultrasound examination after thyroid surgery and ultrasound diagnosis of parathyroid disease, recurrent goiter, and neck masses. Ultrasound-guided minimally invasive techniques, such as fine-needle aspiration biopsy, percutaneous laser ablation, and ethanol and glucocorticoid injections, are considered in depth.
