Abstract

Background: Metastatic breast cancer patients may require both radiotherapy and chemotherapy. A lot of uncertainty exists in clinical practice about the optimal timing of both treatment modalities in this setting. Concurrent chemoradiotherapy may increase efficacy and allow more rapid introduction or continuation of chemotherapy, but may be associated with increased toxicity compared to sequential radiotherapy followed by chemotherapy. Methods: We searched the medical literature for evidence concerning risks and benefits of concurrent versus sequential chemoradiotherapy in metastatic breast cancer with the aim of providing recommendations for routine clinical practice. Results: We found low grade evidence for safe and effective treatment of breast cancer metastases with concomitant radiotherapy and certain chemotherapeutics (e.g. capecitabine, vinorelbine) at a specific dose and in a specific setting (e.g. treatment of bone metastases). Conclusion: Only sparse evidence exists concerning concurrent chemoradiotherapy in metastatic breast cancer. We provide a discussion on important aspects to consider when evaluating a patient for concurrent chemoradiotherapy and summarize sparse evidence concerning specific chemoradiotherapy regimens. More clinical investigations are warranted before more comprehensive guidance to the clinician can be given.
