Abstract

In dit eindwerk gaat het over een gehuwd paar. Beiden zijn zelfstandig. Ze oefenen een vrij beroep uit als advocaat. Aangezien de belastingdruk hoog is in de personenbelasting, bij het uitoefenen van het vrij beroep, heb ik de gegevens van het vrije beroep gesimuleerd in een vennootschapsvorm. Zo worden de inkomsten gespreid in de personen- als in de vennootschapsbelasting. Men kan zo optimaal genieten van de laagste tarieven. Er worden drie situaties bekeken: de eenmanszaak, de niet-KMO-vennootschap en de KMO-vennootschap. Als verdere optimalisering ga ik na wat het resultaat is als men het praktijkgedeelte van de woning verhuurt aan de BVBA. Ik kom tot het besluit dat men door een BVBA op te richten en te optimaliseren, veel minder belastingen moet betalen.

Abstract

Tissue engineering wordt beschouwd als een vernieuwende en veelbelovende strategie voor de behandeling van chronische wonden en brandwonden. Bij deze patiënten zij n hun intrinsieke groeifactorconcentraties sterk verminderd en gespecialiseerd e cellen afwezig. We veronderstellen dat cellulaire therapie en gentransfe r van groeifactoren het weefselherstel van diepe huiddefecten stimuleren en verbeteren. Volgens onze benadering zorgen geëxpandeerde cellen voor het noodzakelijk weefselsubstraat, de gentransfer voor de vereiste ‘intellig entie’ en de biomatrix voor de onmisbare sturing in de helende wonde. Een voordien ontworpen en gestandaardiseerd varkensmodel werd gebruikt als preklinisc h model om diepe huiddefecten te simuleren en een natte wondomgeving te creëren door speciaal ontworpen wondkamers. Vooreerst brachten we basale keratinocyten, genetisch gemanipuleerd met VEGF met b ehulp van ex vivo liposomale transfer onder de controle van tetracycline, in diepe wonden met doel op verbeterde wondheling. Een hoge VEGF concentratie werd verkregen door toevoeging va n 1µg/ml tetracycline en bevorderde de fibronectine depositie, vascularisa tie en reëpithelisatie. Vervolgens optimaliseerden we verschillende niet-virale gentransfermethoden voor epidermale en dermale doeleinden. Fugene HD is te verkiezen als transfer methode voor epidermale studies, terwijl Effectene het meest optimaal reagentia voor angiogene studies blijkt te zijn. Transfectie met behulp van nucleofecti e wordt verkozen bij fibroblasten als genetische carrier. Algemeen gezien is dez e het meest optimaal voor ex vivo gentransfer voor wondheling of tissue engineering van de huid. In de zoektocht naar een lichaamseigen huidsubstituut worden diepe wonden beha ndeld met een autologe plaatjes-rijke plasmagel, gesupplementeerd met endothel iale progenitorcellen en basale keratinocyten. Deze plasmagel treedt op als e en ondersteunende biomatrix en, samen met de toevoeging van specifieke cell en, verbetert ze de organisatie van de extracellulaire matrix, vascularisatie en wonds luiting. Om de volledige huid te reconstrueren moet ook de onderliggende subcutislaag a angepakt worden. Processed lipoaspirates zijn geschikt om deze laag opnieuw te be volken omwille van hun fibroblastachtige morfologie, hun intrinsieke adipogene karakter en hun mesenchymale differentiatiecapaciteit. Deze projecten vormen momenteel de ruggengraat voor autologe tissue engineering van de huid en dragen bij tot de potentiële ontwikkeling van een 3 dimensionaal gevasculariseerd huidequivale

Abstract

Introduction: Breast reduction surgery can be performed using different pedicles. So far no single technique has been proven to be superior to all others. At the University Hospital of Leuven we preferably use the superomedial pedicle technique since 2006. Aim: To evaluate patient satisfaction, nipple sensitivity and the complication rate in our center. To determine potential predictors of postoperative success and to evaluate breast feeding capacity in women becoming pregnant after surgery. Patients & methods: We retrospectively analyzed all 479 patients that underwent breast reduction surgery with a superomedial pedicle in our center, between March 2006 and May 2015. Between 6 months and 4 years postoperatively all patients received a survey questioning nipple sensitivity, satisfaction with their bosom, shape and size immediately after surgery and at various time points postoperatively. Electronic patient files were systematically reviewed for elementary baseline characteristics and postoperative complications. Women becoming pregnant after breast reduction surgery were questioned about their breastfeeding capacity. Results: 259 (54%) patients filled in the survey and form the study cohort. Symptoms of inframammary skin irritation, neck and shoulder pain decrease significantly after surgery. At the moment of questioning nipple sensitivity remained unaffected or had improved in almost 80% of the participants. More than 75% were satisfied or very satisfied with their breast size, shape and bosom. Postoperative complications are comparable with those described in the literature and most tend to resolve within the first 6 months. A higher BMI and a higher amount of resected tissue were significant predictors of postoperative complications. Breast feeding was successful in 1 of the 4 women that became pregnant after surgery. Discussion: In our hands the superomedial pedicle technique is a safe procedure for breast reduction. Patient satisfaction is high and complication rates are similar to those described in the literature. Breastfeeding was successful in 1 patient. Due to the very limited number of postoperative pregnancies it is however impossible to draw firm conclusions on breast feeding potential in general.

Spontaneous and dynamic smile restoration after long-standing facial paralysis in adults: A Systematic Review

Abstract

Background: For longstanding facial paralysis, gracilis free muscle transfer (GFMT) is a frequently used free muscle flap to reconstruct the smile. In recent years a dual-innervation technique, consisting of a cross-facial nerve graft (CFNG) along with masseteric nerve (NM) coaptation was introduced. Theoretically, this approach could provide a marriage between strength and spontaneity of the smile. In this review, we investigated the benefit of GFMT with dual-innervation in comparison to single innervation focusing on smile spontaneity. Methods: A systematic review of literature was performed in accordance with the PRISMA guidelines. We searched MEDLINE, Embase and Cochrane library from 1995 to 2021 using well-chosen keywords adapted to each database. Abstracts mentioning surgical treatment of facial paralysis were screened. Eligibility criteria were the use of a GFMT and at least one patient group treated with dually innervated GFMT. Results: We retrieved 568 records of which 12 studies were found eligible. In the total of 12 studies, 341 patients were treated with GFMT. Of these 12 eligible studies, 5 had only one patient group treated with dual innervation whereas 7 studies included at least a second patient group with either the CFNG (n=3), the MN (n=4) or one of both (n=1) as the single neurotizer. Dual GFMT innervation resulted in a spontaneous smile in the majority of patients and seemed the preferred method over single innervation methods although analysis is highly hampered by the heterogeneity of measuring and reporting. Conclusion: Our review demonstrates that dually innervated GFMT may be the preferred method for spontaneous smile restoration in adult patients. The most striking finding is however, the lack of standardization in measuring and reporting smile characteristics. The use of a fully standardized surgical procedure with standardized data collection and outcome analysis, is required to conclusively answer the questions investigated in this review.

Integra, still not beaten by the next generation skin substitutes?

Abstract

Background: Skin substitutes are successful treatments for full-thickness skin lesions. For many years Integra has been considered the ‘gold-standard’ among dermal skin substitutes in clinic, but it is not clear if this is still the case to date. Objectives: This review tried to figure out if Integra is still the best bioengineered dermal skin substitute on the market to date for the treatment of full-thickness skin lesions. Methods: A systematic review was carried out in October 2019 to identify all relevant articles. Pubmed has been used as database and both trials and case series were considered. We compared the skin substitutes’ performances in different clinical outcome categories. Results: This review included a total of 77 articles of which 25 randomized controlled trials. These articles show that Integra double layer is still superior with regard to the healing rate and the infection incidence in diabetic foot ulcers. Also concerning the contracture incidence in acute wounds Integra still scores the best results. On the other hand it is Apligraf, which showed superior results concerning the healing rate and hematoma/seroma incidence in acute wounds. In addition, Apligraf showed similar results compared to Integra double layer concerning the infection incidence in acute wounds and the final cosmetic outcome of the scars. The Integra dermal matrix covered with a silicone top layer is the most commonly used Integra-variant, although this dermal matrix is also available without this silicone layer. Surprisingly enough, this single layer Integra-variant shows similar (concerning the infection incidence in acute wounds and the final aesthetic outcome) and even better (concerning the hematoma/seroma incidence) clinical results compared to Integra double layer. Together with the fact that Integra single layer is probably cheaper due to the absence of the silicone layer and more efficient because the silicone layer does not have to be removed in a second operation, these results might question the function of this silicone top layer. Conclusion: There are some definite indices that Integra DL might not be such a victorious winner as once considered. Serious SS competitors, such as Apligraf, might beat Integra DL after all. But many more randomized clinical trials are still needed to confirm or refute these conclusions.
