Chronic postoperative pain is a major issue in many surgeries.Depending on the type of the surgery, it occurs in in 5% to 80% of individuals. Intraoperative hypnosis is a potential preventive method which could reduce the incidence of chronic postoperative pain. While the positive effect of hypnosis on acute postoperative pain has already been proven, its benefit on the incidence of chronic postoperative pain remains uncertain.The goal of this pilot study is to compare the incidence of chronic pain between two groups of women operated of breast cancer.The first group was operated by quadrantectomy under general anesthesia while the second group had undergone the same operation, but under local anesthesia and intraoperative hypnosis. The selection of the samples was conducted retrospectively, based on medical records. The data regarding chronic pain have then been evaluated through a questionnaire addressed to the patients. To consider the potential effect of confounding factors, a logistic regression analysis was made.Of the 266 patients selected, 183 responses were registered. The relative risk for the development of chronic pain in the group operated under general anesthesia (59% of chronic pain) as compared with the group operated under local anesthesia and hypnosis (56% of chronic pain) was 1.05 (95% IC, O.77 to 1.42, p-value=O .77). The adjusted relative risk obtained by the logistic regression analysis was 0.87 (95% IC, 0.40 to 1.89). This study doesn't show any significant difference in terms of chronic pain between the group operated under general anesthesia and the group operated under local anesthesia and hypnosis. This result is found even after adjustment by the logistic regression analysis. Nevertheless, selection and informations biases are present in this study, notably due to the retrospective character of the study. Les douleurs chroniques post-opératoires constituent un problème majeur dans un grand nombre de chirurgies. Leur incidence est de 5 à 80% selon le type de chirurgie considéré. Face à ce problème, l'hypnose per-opératoire constitue une méthode susceptible de pouvoir réduire l'incidence de ces douleurs chroniques. Si son effet bénéfique sur l'incidence de douleurs aiguës post-opératoires est bien connu, son effet sur l'incidence des douleurs chroniques post-opératoires reste incertain.Méthodes: L'objectif de cette étude pilote est de comparer l'incidence de douleurs chroniques auprès de deux échantillons de femmes opérées pour un cancer du sein. Le premier échantillon a été opéré par quadrantectomie sous anesthésie générale tandis que le second a subi la même opération, mais sous anesthésie locale avec hypnose per-opératoire. La sélection des échantillons a été réalisée de manière rétrospective, sur base de dossiers médicaux. Les données concernant la présence de douleurs chroniques ont ensuite été évaluées par un questionnaire envoyé aux patientes. Pour tenir compte des éventuels facteurs de confusion, une analyse de régression logistique a été effectuée.Résultats: Sur les 266 patientes sélectionnées, 183 réponses ont été enregistrées. Le risque relatif de développer des douleurs chroniques dans le groupe opéré sous anesthésie générale (59% de douleurs chroniques) comparé au groupe opéré sous anesthésie locale et hypnose (56% de douleurs chroniques) est de 1.05 (IC à 95% entre 0.77 et 1.42, p-value=0.77). La valeur du risque relatif ajusté obtenue par analyse de régression logistique est de 0.87 (IC à 95% entre 0.40 et 1.89).Conclusions : Cette étude ne montre pas de différence significative au mveau de l'incidence de douleurs chroniques entre le groupe opéré sous anesthésie générale et le groupe opéré sous anesthésie locale et hypnose. Ce résultat est retrouvé même après ajustement par l'analyse de régression logistique. Néanmoins, des biais de sélection et d'information sont présents, notamment à cause du caractère rétrospectif de l'étude.
