The NIRS technology is a technic of spectroscopy in the near infrared of the light spectrum. The first apply in the medical field has taken place in the beginning of the 90's. This type of technology leads to a new scientific advance because it offers a continuous non-invasive monitoring at the patient's bedside. Moreover,it is related with the perfusion of organs like the brain, the liver, the kidney or the guts. The monitoring is based on electrodes emitting signal through the scalp and the skull. lt provides accurate information about the ratio between oxygen supply and its consumption. This kind of technology couId lead to breakthrough information, for instance combined to a vaso-occlusive test in the support of a perioperative choc.The population approached in the present study is the frail and the elderly patient undergoing heavy orthopaedic surgery . This type of patient is highly exposed to suffer from postoperative conditions.Statistical analysis tends to daim that the length of stay of the severe frai! group is significantly longer that the average length of hospitalization of the non-frail group. Besides, the rS02 baseline seems to be useful to assess the frailty of a new inpatient. lndeed, baselines before inducing tends to be significantly lower in the severely frail group compared to the non-frail group. The results suggest that a correlation between a low rS02 baseline and a long length of stay in the elderly severely frail group could be investigate in a forthcoming work.NIRS monitoring has revealed efficiency to detect anaemia during the perioperative period. lt seems to provide an early detection of anaemia in comparison with the classical monitoring{BPM, BP, Sp02). Furthermore, it appears to be related with the blood gases which were performed at the same time (Hb, lactic acid,...). The use of NIRS monitoring could represent an additional decisional factor to perform or not a blood transfusion in the elderly and frail population. Eventually, that could impact on the recovery and the onset of postoperative conditions in the elderly and frail population. La technologie NIRS est une technique de spectroscopie dans le proche infrarouge. Ses premières applications médicales datent des années 90. Elle constitue une avancée car elle permet un monitoring continu et non invasif au lit du patient, spécifique de la perfusion de certains organes tels que lecerveau, le foie, les reins ou les intestins. Le monitoring se réalise à l'aide d'électrodes émettant généralement à travers le scalp et le crâne, et fournissant des informations précises sur le ratio entre l'approvisionnement d'oxygène et son utilisation. Cette technologie pourrait notamment être utile en période de choc peropératoire, combinée à un test vaso-occlusif.Dans cette recherche, nous nous sommes intéressés à la personne âgée fragile subissant des opérations lourdes de type orthopédique. Cette partie de la population est à risque de présenter des complications postopératoires.Les analyses statistiques tendent à montrer que la durée de séjour des patients sévèrement fragiles est significativement plus élevée que la durée moyenne d'hospitalisation des patients non fragiles. De plus, les valeurs rS02 semblent intéressantes comme mesure de la fragilité d'un patient. En effet, les seuils de bases rS02 à l'induction se révèlent significativement inférieurs dans la population sévèrement fragile par rapport à la population non fragile. Les résultats suggèrent qu'une corrélation entre un niveau bas au seuil de base rS02 et une durée de séjour élevée chez un patient âgé fragile pourrait être investiguée dans le futur.Le monitoring NIRS s'est également révélé efficace dans la détection précoce d'anémie peropératoire par rapport au monitoring classique (FC, TA, Sp02). Il s'accorde avec les gazométries réalisées peropératoires (Hb,lactate,...). L'utilisation de la NIRS pourrait donc représenter un facteur décisionnel supplémentaire dans la mise en place d'une transfusion sanguine, impactant sur la récupération per et postopératoire de la personne âgée fragile.
