Introduction : les carcinomes épidermoïdes de la tête et du cou représentent le 6ième cancer le plus fréquent du monde. Dans 90% de ces tumeurs, nous observons une surexpression d’EGFR (epidermal growth factor). De ce fait, le cetuximab, un anticorps monoclonal de l’EFGR semble un moyen thérapeutique intéressant. Néanmoins, les bénéfices de cette molécule sont limités en termes de suivi global. L’étude extrême montre que le groupe de patients sous Cetuximab et chimiothérapie, a une survie médiane globale et une survie sans progression de 10,1 mois et de 5,6 mois respectivement, comparé au groupe sous chimiothérapie seule, où ces valeurs sont de 7,4 mois et 3,3 mois. Une nouvelle voie de signalisation a suscité l’intérêt des scientifiques à savoir, la voie d’EGFR-GEP100-Arf6-AMAP1-cortactine. De fait, la cortactine est surexprimée dans 30% des cancers de la tête et du cou. Le but de ce mémoire est d’analyser, si cette voie a un impact sur la réponse au traitement à base de cetuximab chez les patients en situation palliative atteints d’un carcinome épidermoïde de la région de la tête et du cou. Matériels et méthodes : Dans cette étude rétrospective, nous avons utilisé les données de patients atteints d’un carcinome épidermoïde des voies aéro-digestives supérieures, en récidive et inopérable et/ou métastatique. Toutes les données ont été rassemblées entre 2014 et 2017 et proviennent de patients traités dans le service d’oncologie des Cliniques universitaires Saint-Luc. Ainsi, nous avons eu accès aux données de 117 patients. Parmi ces 117 patients, 48 patients ont bénéficié du traitement qui nous intéresse. Celui-ci est composé de cetuximab cisplatine et du 5fluprouracile. Nous avons alors constitué 2 groupes de patients se distinguant par leur survie sans progression. Le 1er groupe est composé de 25 patients et correspond aux répondeurs courts qui se définissent par une survie sans progression de moins de 6 mois. Le 2ième groupe est composé de 23 patients, répondeurs longs, qui eux n’ont pas progressé dans les 6 mois de traitement. Les blocs et lames des biopsies ou pièces opératoires de 9 patients répondeurs courts et de 11 patients répondeurs longs ont été mis à notre disposition. L’expression immunohistochimique du pEGFR et de la cortactine a été évaluée par histoscore sur les échantillons disponibles de ces 2 marqueurs sur la survie a été évaluée. Résultats : Nous avons utilisé les données des 20 patients, pour lesquels on dispose de blocs et lames de biopsie ou pièces opératoires. On observe une survie médiane de 10,46 mois avec un IC95 (3, 98 -15,63) et une survie sans progression médiane de 7,86 mois avec un IC95 (2,34 – 13,42). Ni le taux de la cortactine, ni le taux de pEGFR n’ont montré une influence sur la survie globale (HR de 0,99) ou sur la survie sans progression (HR de 0,99) ou sur la survie sans progression (HR de 0,99 pour toutes les variables). Conclusion : Dans notre étude, la voie de signalisation d’EGFR-GEP100-Arf6-AMAP1-cortactine n’a pas d’influence sur la survie globale ou la survie sans progression. Background: The head and neck squamous cell cancer is the sixth most common cancer worldwide. Up to 90 % of patient’s overexoress EGDR. Therefore, Cetuximab, a monoclonal antibody blocking the EGFR activation seems to be a good target. Nevertheless, the benefit on overall survival is limited. The extreme study showed that patients receiving Cetuximab and chemotherapy had an overall if 10, 1 month and a progression free survival of 5,6 month compared with the group without Cetuximab having an overall survival of 7,4 and a progression free survival of 3.3 month. A new signaling pathway, EGFR-GEP-100-Arf6-AMAP1-cortaction, grabbed the interest of scientists. Additionnally, cortactin is overexpressed in 30% of head and neck cancer. The aim of this study is to analyze if some factors could have the capacity to influence the response of patients to the chemotherapy Cisplatine, 5-Fluorouracil and Cetuximab. The most interesting factor is the expression level of cortactine and PEGFR. Materials and methods: To accomplish this retrospective study, we focus on head and neck squamous cell carcinoma, more precisely on recurrent and inoperable and/or metastatic disease. We gathered the information of 117 patients from 2014 to 2017. Those patients were treated in the department of medical oncology at the university hospitals “ Saint Luc”. Only 48 patients were treated with the chemotherapy on which we will focus. This chemotherapy comprise cetuximab in combination with cisplatine and 5 fluoriracil distinguished 2 groups, different from each other, due to the progression free survival. The first group counts 23 patients with no progression in the first 6 month of treatment. The second group is composed from 25 patients progressing after less than 6 month of chemotherapy. Afterwards, we had access to biopsy samples or the resected tumors. We found 11 tumors for the first group and 9 specimens for the second group. PEGFR and cortactin were analyzed by immunohistochemistry and the intensity of staining was evaluated with the establishment of a histoscore. Results: the data from the 20 patients with biopsy samples or resected tumors has been used to analyze the influence of staining rate of cortactine and PEGFR on overall survival and progression free survival. The median overall survival was 10, 46 month with an IC95 (3,98 -15,63) and the median progression free survival was 7,86 months with IC95 (2,34-13,42). Neither the cortactine rate, nor the PEGFR rate show a significant influence on overall survival (HR de 0, 99) or progression free survival (HR de 0, 99 for all the values. Conclusion: In our study, the signaling pathway of EGFR-GEP100-Arf6-AMAP1 cortactin has no influence on overall survival or progression free survival.
