Abstract

Lo studio delle malformazioni fetali rappresenta uno degli argomenti più complessi nell’ambito della medicina fetale. Negli ultimi anni il ruolo della risonanza magnetica (RM) nell’inquadramento diagnostico delle malformazioni fetali isolate e complesse è divenuto sempre più importante, attestandosi come esame di III livello. Tale incremento è stato possibile grazie all’implementazione tecnologica delle apparecchiature di RM e quindi allo sviluppo di sequenze veloci e ultra veloci. La RM, solitamente effettuata dopo un’ecografia di II livello a partire dalla 19a settimana di gravidanza, può quindi rappresentare, in casi selezionati, un importante completamento diagnostico nella valutazione di quadri malformativi complessi e nella programmazione di interventi terapeutici nella vita intrauterina e nel postnatale. È necessario sottolineare l’importanza di centri di riferimento dedicati e la presenza di un team multidisciplinare per la corretta gestione dei piccoli pazienti. Il volume è suddiviso in tre parti. Dopo una prima parte più tecnica, nella seconda parte viene trattata l’anatomia fetale in RM ponendo attenzione all’evoluzione dei singoli organi durante la vita fetale; la terza parte analizza invece i principali quadri malformativi con particolare riguardo al distretto encefalico, toracico e addominale, il tutto associato ad un’estesa documentazione iconografica. Il libro vuole essere di rapida e facile consultazione e rappresentare un punto di partenza per il radiologo e il neuroradiologo, nonché per tutti gli specialisti interessati alla medicina fetale e in particolare alla risonanza magnetica.  .

Abstract

Ultrasound (US) prenatal screening has been proposed as the most effective technique for Trisomy 21 early assessment. Assessment of Nuchal Translucency (NT) offers promising non-invasive method for fetal abnormalities detection up to 75%. Nevertheless, current clinician practice of NT examination by locating the sonogram calipers on 2D US image requires highly trained and competent operators by adhering to a standard tedious protocol; therefore it is prone to errors and hence it decreases the reliability in intra- and inter-observer repeatability. This Brief provides the basic knowledge regarding Trisomy 21 diseases and its existing detection methods. The restrictions and disadvantages of each method are discussed accordingly. Therefore, a non-invasive early detection method using 3D ultrasound reconstruction of Nuchal Translucency is introduced. This new method for 3D NT assessments has an edge over the previous 2D methods, and entails the composite function in visualizing the explicit internal marker structure. Further, image processing techniques covered from data acquisition, pre-processing, speckle noise reduction and 3D segmentation are also discussed. This should be especially useful for students and professional researchers in the Biomedical and image processing fields.
