Abstract

Clinical usage of artificial pacing dates back to 1958, when the battery powered cardiac pacemakers became available. Modern implantable pacemakers are the complicated electronic devices operating 10 years continuously without battery exchange. Though the development of devices is not a primary topic of the book, certain efforts towards developing of biologic pacemakers through tissue engineering and studying of cell synchronization are discussed. The main attention is paid to implementations of pacemakers in different medical situations oriented towards widening the clinical indications for implanting the cardiac pacemakers. New methods and devices in cardiac resynchronization therapy (CRT) have received particular attention. Placing of pacing electrodes has been treated soundly. Furthermore, emerging of complexities and complications in new clinical situations and other safety problems have been discussed thoroughly. The authors have derived the used information from their own clinical practice and experiences of their medical colleagues. These and other pragmatic features can be acknowledged as the most valuable asset of the book.

Abstract

The book focuses upon clinical as well as engineering aspects of modern cardiac pacemakers. Modern pacemaker functions, implant techniques, various complications related to implant and complications during follow-up are covered. The issue of interaction between magnetic resonance imaging and pacemakers are well discussed. Chapters are also included discussing the role of pacemakers in congenital and acquired conduction disease. Apart from pacing for bradycardia, the role of pacemakers in cardiac resynchronization therapy has been an important aspect of management of advanced heart failure. The book provides an excellent overview of implantation techniques as well as benefits and limitations of cardiac resynchronization therapy. Pacemaker follow-up with remote monitoring is getting more and more acceptance in clinical practice; therefore, chapters related to various aspects of remote monitoring are also incorporated in the book. The current aspect of cardiac pacemaker physiology and role of cardiac ion channels, as well as the present and future of biopacemakers are included to glimpse into the future management of conductions system diseases. Abstract

Ultra Low-Power Biomedical Signal Processing describes signal processing methodologies and analog integrated circuit techniques for low-power biomedical systems. Physiological signals, such as the electrocardiogram (ECG), the electrocorticogram (ECoG), the electroencephalogram (EEG) and the electromyogram (EMG) are mostly non-stationary. The main difficulty in dealing with biomedical signal processing is that the information of interest is often a combination of features that are well localized temporally (e.g., spikes) and others that are more diffuse (e.g., small oscillations). This requires the use of analysis methods sufficiently versatile to handle events that can be at opposite extremes in terms of their time-frequency localization. Wavelet Transform (WT) has been extensively used in biomedical signal processing, mainly due to the versatility of the wavelet tools. The WT has been shown to be a very efficient tool for local analysis of non-stationary and fast transient signals due to its good estimation of time and frequency (scale) localizations. Being a multi-scale analysis technique, it offers the possibility of selective noise filtering and reliable parameter estimation. Often WT systems employ the discrete wavelet transform, implemented on a digital signal processor. However, in ultra low-power applications such as biomedical implantable devices, it is not suitable to implement the WT by means of digital circuitry due to the relatively high power consumption associated with the required A/D converter. Low-power analog realization of the wavelet transform enables its application in vivo, e.g. in pacemakers, where the wavelet transform provides a means to extremely reliable cardiac signal detection. In Ultra Low-Power Biomedical Signal Processing we present a novel method for implementing signal processing based on WT in an analog way. The methodology presented focuses on the development of ultra low-power analog integrated circuits that implement the required signal processing, taking into account the limitations imposed by an implantable device.
