Abstract

Diabetes is a chronic, incurable disease that affects approximately 5% of the world population. People with type 1 diabetes and in some cases, those with type 2 are processed by a series of daily subcutaneous injections of insulin. This unwieldy and inconvenient treatment can results in a lack of compliance. It is for this reason that many studies are emerging to develop a more appropriate way. For now, the development of oral or pulmonary insulin is unfortunately not successful. A new alternative, the nasal spray, is the subjects of numerous studies and shows promising signs of efficacy. The first study described in this work shows that the formulation of insulin nasal spray created by Bentley Pharmaceuticals is relatively well absorbed. Indeed, after nasal administration of 25 IU of insulin, there was a fall in plasma glucose similar to that observed after subcutaneous administration of 4 IU insulin. The second study addressed in this paper describes the administration of analogue mixtures insulin to fast and slow action on hyperglycemic and hypoinsulinaemic rats. The pharmacokinetic profile reflects the absorption due to quick action and prolonged action of insulin. These promising results encourage future longer studies on a larger scale and on diabetic subjects. Another indication of insulin nasal spray could emerge. Indeed, the third study discussed in this paper demonstrates that the administration of intranasal insulin stabilized or improved cognition, function and cerebral glucose metabolism in adults with amnestic mild cognitive impairment or Alzheimer’s disease. Future larger and longer studies are needed to show the long-term effects of insulin nasal spray for Alzheimer’s disease Le diabète est une maladie chronique incurable qui touche près de 5% de la population mondiale. Les personnes atteintes de diabète de type 1 et dans certains cas, celles atteintes de type 2, sont traitées par une série d’injections sous-cutanées quotidiennes d’insuline. Ce traitement peu commode et peu pratique, peut engendrer un manque de compliance. C’est pour cette raison que de nombreuses études voient le jour afin de développer un moyen plus approprié au quotidien. Pour l’instant, le développement d’une insuline par voie orale ou par voie pulmonaire n’est malheureusement pas fructueux. Une nouvelle alternative, le conditionnement en spray nasal, fait l’objet de nombreuses études et montre des signes prometteurs d’efficacité. La première étude décrite dans ce mémoire montre que la formulation d’insuline en spray nasal créée par Bentley Pharmaceuticals est relativement bien absorbée. En effet, après administration nasale de 25 UI d’insuline, on observe une chute du glucose plasmatique semblable à celle observée après administration sous-cutanée de 4UI d’insuline. La seconde étude abordée dans ce travail décrit l’administration de mélanges d’analogues d’insuline à action rapide et lente sur des rats hyperglycémiques et hypoinsulinémiques. Le profil pharmacocinétique observé reflète bien l’absorption due à une action rapide et à une action prolongée de l’insuline. Ces résultats prometteurs encouragent de futures études plus longues, à plus large échelle et portant sur des sujets diabétiques. Une autre indication à l’insuline en spray nasal pourrait voir le jour. En effet, la troisième étude évoquée dans ce mémoire démontre que l’administration de l’insuline intranasale stabilise ou améliore la cognition, la fonction et le métabolisme cérébral du glucose pour les adultes ayant une déficience cognitive amnésique légère ou la maladie d’Alzheimer. De futures études plus vastes et plus longues seront nécessaires afin de montrer les effets à long terme de l’insuline en spray nasal sur la maladie d’Alzheimer
