Abstract

La carence en vitamines B1, B6 et B12 est un véritable problème de santé publique. En effet, de nombreuses populations sont potentiellement à risque de déficit. Parmi celles-ci, quelques exemples peuvent être cités : les végétariens et les végétaliens, les femmes enceintes et allaitantes, les personnes dénutries, les patients présentant un défaut d'absorption, les patients bariatriques, les alcooliques chroniques, les diabétiques, les patients souffrant d'hypothyroïdie, les insuffisants cardiaques, les hémodialysés chroniques, les patients sous IPP, anti-H2, metformine, diurétique, isoniazide, cyclosérine, pénicillamine, hydralazine, carbidopa. Cette carence peut mener à des conséquences cliniques parfois sévères, souvent négligées. Les recommandations à propos d'une supplémentation chez ces patients à risque sont divergentes. En effet, des guidelines précises sur les modalités de substitution n'existent pas à ce jour. Cependant, de fortes doses de vitamines B1 et B12 n'entraineraient pas d'effets indésirables notables contrairement à la vitamine B6. Les professionnels de la santé doivent donc garder à l'esprit que l'utilisation thérapeutique de vitamine B6 à haute dose n'est pas sans risque et est susceptible d'entrainer des désordres neurologiques. En conclusion, s'intéresser davantage au risque de carence en vitamines B1, B6 et Bl2 et agir en amont des problèmes seraient profitables pour le patient et la société. Vitamins B1, B6 and B12 deficiency are a real public health problem. Indeed, many populations are potentially at risk of deficit. Among these, some examples may be listed: vegetarians and vegans, pregnant and nursing women, malnourished people, patients with a defect of absorption, patients with bariatric surgery, chronic alcoholics, diabetics, and patients with hypothyroidism, patients with heart failure, chronic hemodialysis, patients on PPIS, H2 blockers, metformin, diuretic, isoniazid, cycloserine, penicillamine, hydralazine, and carbidopa. This deficiency can lead to clinical consequences sometimes severe and often neglected. Recommendations about supplementation for these patients at risk are divergent. Indeed, they are no specific guidelines concerning the modalities of substitution so far. However, high doses of vitamins B1 and B12 would not lead to significant side effects unlike vitamin B6. Therefore, health professionals must keep in mind that the therapeutic use of vitamin B6 in high doses is not without risk and is likely to lead to neurological disorders. In conclusion, more interest in the risk of deficiency of vitamins B1, B6 and B12 and act upstream of the problems would be beneficial to the patient and the society.
