The aim of this prospective study is to assess the mortality, the morbidity and the oncology prognostic in patients with peritoneal carcinomatosis from colorectal origin, after complete cytoreductive surgery and hyperthermia intraperitoneal chemotherapy (HIPEC). For a long time, peritoneal carcinomatosis was considered as a terminal condition with poor prognosis.Methods: We prospectively followed a group of patients from 2007 to 2012. Those patients were suffering from peritoneal carcinomatosis from colorectal origin and they all underwent complete cytoreductive surgery and HIPEC with the closed abdomen technique. The Oxaliplatine was the drug used for most of the procedures.Results: The study involved 37 surgical procedures in 37 different patients (F/M: 19/18). Mortality rate is 3% (N=1). Surgical complications occurred in twenty patients (54%) and four of them (11%) required surgical revision within the 30-day postoperative period. The number of removed organs significantly increases the occurrence of postoperative complications (Odds ratio: 2.09 (1.19-4.57), p-value<0.05). Median disease free survival (DFS) is 14.3 months and the DFS at 1 year and 3 years is respectively 58.7 % and 18.6%. Overall survival (OS) at 1 year and 3 years is respectively 90.4 % and 73.4%. Multivariate analysis demonstrates involved nodes as a factor affecting DFS (hazard ratio : 3.67 (0.94-14.36), p-value = 0.05).Conclusions : Although the DFS is still low, our study shows a long overall survival. The rates of mortality and morbidity are acceptable.The results of our study and those from medical literature show there is no argument to deny surgical resections plus HIPEC with oxaliplatin to patients suffering from peritoneal carcinomatosis from colorectal origin if there is a valid selection. Cette étude prospective a pour but d'évaluer la mortalité, la morbidité et le pronostic oncologique des patients atteints de carcinose péritonéale d'origine colorectale, ayant bénéficié d'un traitement combinant la résection chirurgicale des implants de carcinose et une chimiothérapie hyperthermique intrapéritonéale à base d'oxaliplatine. En effet, jusqu'à l'apparition de ce nouveau type de traitement , la carcinose péritonéale était considérée comme un stade métastatique terminal avec, par conséquent un sombre pronostic. Méthodes: Les données ont été collectées de manière prospective et l'étude porte sur une période allant d'octobre 2007 à septembre 2012. Elle concerne un groupe de patients atteint de carcinose péritonéale d'origine colorectale et traité par chirurgie de résection radicale des lésions (cytoréduction) associée à une chimiohypertherpie intrapéritonéale à l'oxaliplatine, selon le procédé dit « à ventre fermé ». Résultats : Trente-sept interventions chirurgicales suivies d'une CHIP ont été réalisées chez 37 patients différents (F/H : 19/18). Le taux de mortalité postopératoire s'élève à 3% (N=1). Vingt patients (54%) ont présenté des complications postopératoires dont 11% (N=4) ont dû avoir une réintervention chirurgicale. Le facteur statistiquement significatif qui influence la survenue de complications postopératoires est le nombre d'organes réséqués durant l'intervention chirurgicale (rapport de cotes : 2.09 (1.19- 4.57), p < 0.05). La survie médiane sans récidive s'élève à 14.3 mois. Le taux de survie sans récidive à 1 an et 3 ans est de 58.7 % et de 18.6%, respectivement. Le taux de survie globale à 1 an et 3 ans est respectivement de 90.4 % et de 73.4 %. Le facteur qui influence la survie sans récidive dans notre étude est la présence de ganglions envahis (rapport de taux d'incidence : 3.67 (0.94-14.36), p = 0.05).Conclusion : Bien que la survie sans récidive soit relativement faible, notre étude a mis en évidence une survie globale prolongée. De plus, la présence d'une faible mortalité postopératoire et d'une morbidité acceptable justifient la prise en charge de patients bien sélectionnés, atteints de carcinose péritonéale d'origine colorectale, par cette technique qui combine cytoréduction et chimiothérapie intrapéritonéale à l'oxaliplatine.
