Abstract

Direct oral anticoagulants (DOA) have been placed on the market as an alternative to VKA in prevention and treatment of thromboembolic diseases. The reason is mainly to address a major health problem caused by the VKA: a significant risk of fatal bleeding, mainly intracranial. Even if this new class of anticoagulant drugs has shown some benefits, the issue of hemorrhage remains important. This raises the question of the interest of an antidote that would act rapidly and specifically to antagonize the anticoagulant effect. For example, in cases where the patients have a bleeding that can be life threatening or in cases of urgent surgical operations representing a high risk of hemorrhage. Three antidotes are currently being developed to meet that limit. Idarucizumab, antidote of dabigatran, andexanet alfa, antidote of inhibitors of factor Xa, and ciraprantag, universal antidote. The purpose of this thesis is to write a literature review evaluating the effectiveness and safety of those three antidotes and the potential added value that they bring in the management of bleeding and urgent surgeries under DOA. Idaruzimab has already been introduced in management guidelines. Andexaner alfa and the ciraprantag have also shown encouraging results. However, they require further researchs their introduction on the market. Les anticoagulants oraux directs (AOD) ont été mis sur le marché en alternative aux AVK dans la prévention et le traitement des maladies thromboemboliques essentiellement pour répondre à un problème de santé posé par ces derniers : un risque important de saignements mortels, essentiellement intracrâniens. Si cette nouvelle classe a montré un léger bénéfice, la problématique d’hémorragies reste de mise. Cela a donc mené à se poser la question de l’intérêt d’un antidote qui agirait de façon rapide et spécifique pour antagoniser l’effet anticoagulant dans des cas où les patients présentent un saignement menaçant leur pronostic vital ou en cas de chirurgies urgentes à risque hémorragique élevé 3 antidotes sont actuellement en développement afin de répondre à cette limite. L’idarucizumab, antidote du dabigatran, l’andexanet alfa, antidote des inhibiteurs du facteur Xa et le ciraprantag, antidote universel. Le but de ce mémoire est d’effectuer une revue de la littérature permettant d’évaluer l’efficacité et la sécurité de ces 3 antidotes ainsi que la plus-value éventuelle que ceux-ci apportent dans la prise en charge des saignements et chirurgies urgentes sous AOD. Si l’idarucizumab a déjà été introduit dans des guidelines de prise en charge, l’andexanet alfa et le ciraprantag, bien qu’ils aient présenté des résultats encourageants, nécessitent des études complémentaires avant leur mise sur le marché.
