Abstract

Les thérapies engages suite au diagnostic d’un cancer du sein chez les femmes sont difficiles à supporter. Les traitements sont lourds et contraignants. L’acte pharmaceutique est indispensable dans l’accompagnement du traitement. Différents cas énoncés dans ce mémoire montrent la nécessité d’inclure le pharmacien d’officine dans les soins de support des cancers du sein. Proche de ses patientes, il les sensibilise à une bonne compliance. Il contribue à améliorer leur qualité de vie en leur conseillant des produits de santé adaptés. Nous présentons l’apport bénéfique des conseils du pharmacien dans les mucites, le syndrome main-pied, les bouffées de chaleur et les douleurs dues à l’altération du métabolisme osseux. Les compléments alimentaires sont aussi absorbés. Le pharmacien d’officine s’assure de l’innocuité des produits et de l’absence d’interactions avec le traitement en cours. Seulement le pharmacien, et parfois la patiente, ne dominent pas le traitement engagé à l’hôpital. Dès lors, nous suggérons la mise en place d’un carnet de liaison entre les professionnels de santé et la patiente. Therapies employed during a breast cancer can be difficult to undertake. Treatments are often heavy and binding. The pharmaceutical act is essential in guiding treatment. Different concret cases, established in this study, show that pharmacist cannot be excluded from these supportive cares.Being close to their parents, he sensitizes them to a good compliance. He contributes to improve their life quality by suggesting other adapted health products. We present the beneficial contribution of different advises from the pharmacist in cases of mucositis, hand-foot syndrome, hot flashes and pains due to bone metabolism. Dietary complements have also been talked about. The pharmacist has to be sure of the product’s safety and the lack of interference with an ongoing treatment. Only the pharmacist, and sometimes the patient do not know the treatment given by the hospital. That’s why a proposal establishing a liaison book between health professionals and the patient is suggested.

Abstract

Medication use is the predominant form of health intervention in our society. And as we age, the likelihood of medication use increases dramatically, with more than 80 percent of those over age 65 using one or more medications. Along with that, the potential for medication errors also increases. Indeed adverse drug reactions (ADRs) and adverse drug events (ADEs) are a significant problem in older adults. Written in a practical format by contributors from Australia and the United States, Medication Management in Older Adults: A Concise Guide for Clinicians presents the available evidence on research interventions designed to reduce the incidence of medication errors in older adults, with a focus on acute, subacute, and residential (long-term) care settings.
