Abstract

Background: Hidradenitis suppurativa (HS), also known as acne inversa, is a chronic, recurrent inflammatory disease affecting predominantly skin folds carrying terminal hairs and apocrine glands. Patients and method: In this report, we present our experience of two overwhelmed hidradenitis suppurativa cases with extended perineal lesions treated with abdominoperineal resection followed by reconstruction using a pedicled rectus abdominis musculocutaneous flap (Taylor’s flap). Result: In the immediate post-operative period, the donor-site suffered from eventration and abscesses in one of our patients, which were taken care of. The flap suffered of venous congestion in one case. Local care and the usage of Vacuum-Assisted Closure (VAC) were used and showed good improvement. The post-operative hospitalization period lasted respectively 22 days and 1 month. After a follow-up of more than two and three years, we managed to reduce significantly the comorbidities of our patients. Conclusion: Our treatment method is a reliable surgical option for the radical treatment of overwhelmed hidradenitis suppurativa lesions.

Abstract

De nos jours, les infections compliquées de la peau et des tissus mous constituent un important challenge clinique. S. aureus est le pathogène le plus souvent responsable de ces infections. L’émergence de staphylocoques dorés résistants à la méthicilline (MRSA) nous oblige à redoubler d’efforts pour découvrir et développer de nouveaux agents efficaces contre les infections causées pas de MRSA. La ceftaroline est une nouvelle céphalosporine a large spectre, approuvée en octobre 2010 par la FDA (Food and Drug Administration). Contrairement aux autres β-lactames, la ceftaroline démontre une bonne activité contre les MRSA avec des valeurs de CMI50% et CMI90% (Concentration Minimal Inhibitrice pour 50% et 90% des organismes) de 1mg/L et 2mg/L respectivement. Dans le traitement des infections compliquées de la peau et des tissus mous, la ceftaroline est administrée par voie intraveineuse sous sa forme prodrug, le ceftaroline fosamil, à une dose de 600mg pendant 60 minutes toutes les 12h. La ceftaroline constitue un antibiotique prometteur et bienvenu dans l’arsenal thérapeutique contre les ifnections compliquées de la peau et des tissus mous. Le mode d’action, les propriétés pharmacologiques et microbiologiques de la ceftaroline sont passés en revue dans ce mémoire de fin d’études Nowadays, complicated skin and skin-structure infections (cSSSIs) continue to pose a significant clinical challenge. S. aureus is the most frequently pathogen involved. The emergence of methicillin-resistant staphylococcus aureus (MRSA) has led to increased efforts to discover and develop new agents that are effective against infectins caused by MRSA. Ceftaroline is a new broad-spectrum cephalosporin that was FDA (Food and Drug Adminstration)-approved in October 2010. In contrast with other β-lactams, ceftaroline demonstrates good activity against MRSA with MIC50% and MIC90% (Minimal Inhibitory Concentration for 50% and 90% of organisms) values 1mg/L and 2mg/L, respectively. For treatment of complicated skin and skin structures infections, ceftaroline is administrated by intravenous (i.v.) infusion as the prodrug, ceftaroline fosamil with a dose of 600mg over 60 minutes every 12h. Ceftaroline is a promising antibiotic agent and a welcome addition to our therapeutic armamentarium against complicated skin and skin structures infections. The mode of action, pharmacological and microbiological properties of ceftaroline are reviewed in this Master’s thesis
